| Safety Significant Findings One White Finding associated with failure to: wear dosimeters at the highest exposed portion of the body and use an NRC approved method to determine the effective dose for workers wearing dosimeters on several areas of their bodies. Corrective actions completed by the licensee included re-calculating dose to the affected workers, informing the workers of their revised dose, and verifying that no worker's dose exceeded any federal limits. Palisades will receive increased oversight and an additional inspection using Inspection Procedure 95001. 8 IP 95001 Supplemental Inspection The objectives of an IP 95001 inspection are to provide assurance that: the cause(s) of the deficiency are understood the extent of the issue has been identified corrective actions will prevent recurrence These inspections typically involve 1-2 inspectors and require about 40 inspection hours of effort. This inspection is expected to be performed later this year. 9 Safety Conscious Work Environment (SCWE) Safety Conscious Work Environment (SCWE) - an environment where personnel feel free to raise safety concerns without fear of retaliation, intimidation, harassment, or discrimination. All NRC licensees and contractors are expected to establish and maintain a SCWE which contributes to the safe operation of NRC-regulated facilities. 10 Security Department SCWE Inspections In December 2013, the NRC identified a chilled work environment in the Security Department. Licensee developed SCWE Action Plan. In May 2014, the NRC determined that the quality of licensee actions were insufficient to assess and understand the cause of the chilled SCWE. Licensee enhanced the SCWE Action Plan. In December 2014, the NRC completed a SCWE follow-up inspection which determined a chilled work environment no longer existed in the Security Department. 11 Follow-up SCWE Inspection The focused SCWE inspection conducted in December 2014 concluded that although the Palisades Security Department no longer had a chilled work environment; the cultural improvements remained vulnerable to events that might be perceived negatively by the security officers. The NRC will perform a follow up Problem Identification and Resolution inspection sample in late 2015 to assess whether the improvements in the area of SCWE have been sustained. 12 Other Major Activities in 2015 Triennial Fire Protection Inspection (In Progress) Radiation Protection Inspections (Several weeks throughout the year) Independent Spent Fuel Storage Installation (ISFSI) Inspection (June) Mitigating Strategies Audit (June) Triennial Heat Sink Inspection (November) 13 14 Palisades Nuclear Plant Annual Assessment Summary January 1 - December 31, 2014 Entergy operated Palisades Nuclear Plant in a manner that preserved public health and safety A White finding associated with the failure to adequately assess the radiation dose to workers will result in increased NRC inspection 15 Contacting the NRC Report an emergency (301) 816-5100 (call collect) Report a safety concern (800) 695-7403 or e-mail: Allegation@nrc.gov NRC Resident Inspectors at Palisades: (269) 764-8971 Meeting Feedback General information or questions www.nrc.gov NRC Social Media Channels Blog: http://public-blog.nrc-gateway.gov/ Flickr: http://www.flickr.com/photos/nrcgov/ Twitter: https://twitter.com/#!/nrcgov YouTube: http://www.youtube.com/user/NRCgov RSS: http://www.nrc.gov/public- involve/listserver.html#rss 16 17 Reference Sources Reactor Oversight Process http://www.nrc.gov/NRR/OVERSIGHT/ ASSESS/index.html Public Electronic Reading Room http://www.nrc.gov/reading-rm.html NRC Information Digest http://www.nrc.gov/reading-rm/doc-collections/nuregs/staff/sr1350/ Public Document Room (800) 397- 4209 (Toll Free) 18 Licensee Response and Remarks Entergy Nuclear Operations Palisades Nuclear Plant Remarks Closing Remarks Jack Giessner Deputy Division Director Division of Reactor Projects NRC Region III 19}} | | Opening Eric Duncan Branch Chief, Palisades NRC Region III 2 |

NRC's Annual Performance Assessment of Palisades for 2014 Nuclear Regulatory Commission - Region III South Haven, MI April 9, 2015 1
Opening Eric Duncan Branch Chief, Palisades NRC Region III 2
Purpose and Agenda
- Discuss Palisades Performance in 2014
- NRC Available to Address Questions 3
NRC Mission To license and regulate the nations civilian use of byproduct, source, and special nuclear materials to ensure adequate protection of public health and safety, promote the common defense and security, and protect the environment.
4
Significance Threshold Performance Indicators Green: Baseline Inspection White: Increases NRC oversight Yellow: Increases NRC oversight Red: Increases NRC oversight Inspection Findings Green: Very low safety issue White: Low to moderate safety issue Yellow: Substantial safety issue Red: High safety issue 5

2014 Annual Assessment January 1 - December 31, 2014
- Palisades transitioned to the Regulatory Response column of the ROP Action Matrix in the fourth quarter of 2014 as a result of one finding of low to moderate safety significance (White).
- All additional findings and all performance indicators were Green.
7
Safety Significant Findings
- One White Finding associated with failure to:
- wear dosimeters at the highest exposed portion of the body and
- use an NRC approved method to determine the effective dose for workers wearing dosimeters on several areas of their bodies.
- Corrective actions completed by the licensee included re-calculating dose to the affected workers, informing the workers of their revised dose, and verifying that no worker's dose exceeded any federal limits.
8
IP 95001 Supplemental Inspection
- The objectives of an IP 95001 inspection are to provide assurance that:
- the cause(s) of the deficiency are understood
- the extent of the issue has been identified
- corrective actions will prevent recurrence
- These inspections typically involve 1-2 inspectors and require about 40 inspection hours of effort.
- This inspection is expected to be performed later this year.
9
Safety Conscious Work Environment (SCWE)
- Safety Conscious Work Environment (SCWE) -
an environment where personnel feel free to raise safety concerns without fear of retaliation, intimidation, harassment, or discrimination.
- All NRC licensees and contractors are expected to establish and maintain a SCWE which contributes to the safe operation of NRC-regulated facilities.
10
Security Department SCWE Inspections
- In December 2013, the NRC identified a chilled work environment in the Security Department.
- Licensee developed SCWE Action Plan.
- In May 2014, the NRC determined that the quality of licensee actions were insufficient to assess and understand the cause of the chilled SCWE.
- Licensee enhanced the SCWE Action Plan.
- In December 2014, the NRC completed a SCWE follow-up inspection which determined a chilled work environment no longer existed in the Security Department.
11
Follow-up SCWE Inspection
- The focused SCWE inspection conducted in December 2014 concluded that although the Palisades Security Department no longer had a chilled work environment; the cultural improvements remained vulnerable to events that might be perceived negatively by the security officers.
- The NRC will perform a follow up Problem Identification and Resolution inspection sample in late 2015 to assess whether the improvements in the area of SCWE have been sustained.
12
Other Major Activities in 2015
- Triennial Fire Protection Inspection (In Progress)
- Radiation Protection Inspections (Several weeks throughout the year)
- Independent Spent Fuel Storage Installation (ISFSI) Inspection (June)
- Mitigating Strategies Audit (June)
- Triennial Heat Sink Inspection (November) 13
Palisades Nuclear Plant Annual Assessment Summary January 1 - December 31, 2014
- Entergy operated Palisades Nuclear Plant in a manner that preserved public health and safety
- A White finding associated with the failure to adequately assess the radiation dose to workers will result in increased NRC inspection 14
