TABLE 1 ITEMS WITH FLAWS OR RELEVANT CONDITIONS THAT REQUIRED EVALUATION FOR CONTINUED SERVICE Examination Item Description Evaluation Description Category and Item Number L-B, L2.20 Tendon Wire CAR 201504832 found condition acceptable. Two wires were found protruding from the anchorage of horizontal tendon 45BA at the B buttress. The issue was only identified after liftoff commenced. The average forces for the field end and shop end were found significantly higher than the required yield strength and therefore the tendon is still able to perform its intended function.

L-B, L2.40 Tendon Grease CAR 201506412 found condition acceptable. Preliminary tendon grease reserve alkalinity (base number) tested lower than the specified minimum. Since the grease tested was an "older" grease type requiring a base number of greater than 0 and all affected samples had a base number greater than 0, the grease is found to be capable of performing its intended function . In addition, two more tests were taken and indicate that the base or alkalinity numbers are well within the acceptance criteria per specification C-1003.
