90-Day Post Outage ISI Letter Enclosed is the Post-Outage (90 day) Summary Report for Inservice Inspection examinations and Repair/Replacement activities conducted from the end of the October 2000 refueling outage (01 R1 6) through the November 2002 refueling outage (Q1 R1 7). This report is due 90 days after the end of Q1 R1 7, which is February 24, 2003. This report documents examinations performed during the third period of the third inspection interval.

Q1 R1 7 was the second outage of the third period of the third inspection interval. The third period is scheduled to end on March 9, 2003. This report is submitted in accordance with the requirements of ASME Section XI, Article IWA-6200, "Requirements."

Outage, Period and Interval Start and End Dates This Inservice Inspection Summary Report addresses examinations performed between the end of the previous refueling outage November 3, 2000 through the end of the current refueling outage. This current refueling outage was the 1 7th refueling outage for Quad Cities Nuclear Power Station Unit One. The outage started November 5, 2002 and was completed November 26, 2002 with the unit returned to service.

This refueling outage was the second outage during the third inspection period of the third inspection interval. The third inspection period of the third inspection interval is scheduled to end on March 9, 2003.

Tests, examinations, repairs and/or replacements associated with ASME Section XI were witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors. Non-ASME Section XI or augmented examinations were not witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors and are marked "Not Required" or "N/A" on the applicable examination reports. The Authorized Nuclear Inservice Inspectors are inspectors of the Hartford Steam Boiler Inspection

& Insurance Company of Connecticut (HSB-CT). The HSB-CT Regional Office is located at 2443 Warrenville Road, Lisle, Illinois, 60532-9871.

Examination Program Requirements The examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components", 1989 Edition, no addenda were implemented for Class 1, 2 and 3 components except where written relief has ýbeen granted by the NRC. Written Relief, in the form of Relief Request CR-33 approving Risk Inservice Inspection (RISI) as an inspection program alternative for Code Categories B-F, B-J, C-F-1 and C-F-2, was utilized during QlR17. The ultrasonic examination areas for RISI components were increased, from the Section XI requirements, to meet the RISI Program.

The ultrasonic examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Appendix VIII, 1995 Edition with Addenda through 1996, as required by 10 CFR 50.55a and amended by the Federal Register Notice 64 FR 51370 dated September 22, 1999 is executed through the Performance Demonstration Initiative (PDI) Program according to the schedule defined in 10 CFR 50.55a(g)(6)(ii)(C), for wrought austenitic piping welds, ferritic piping welds, bolts/studs, reactor vessel welds and full structural overlaid wrought austenitic piping welds.

The examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components," 1992 Edition, with 1992 Addenda were implemented for Class MC components except where written relief has been granted by the NRC.

Quad Cities Nuclear Power Station 4300 Winfield Road Unit 1 Warrenville, IL. 60555 22710 206e Avenue North Commercial Service Date February 18, 1973 Cordova, IL 61242 3.0 Abstract of Examinations, Conditions Noted and Corrective Measures 3.1.Section XI Examinations Summary A combination of ultrasonic (UT), liquid penetrant (PT), magnetic particle (MT) and visual (VT) testing was performed prior to and during the 17 th refueling outage on ISI Program components. Examinations were performed by General Electric (GE) and Exelon Nuclear personnel. ISI inspections comprised of 53 volumetric (38 were RISI, Category R-A welds), 14 surface examinations and 41 visual examinations (9 were due to scope expansion) were performed on Class 1, 2 and RISI components.

The additional 9 visual exams were a result of the as-found cold setting being out of tolerance for component 1024A-W-201. The scope was expanded per Code Case N 491-1. Corrective actions, regarding the out of tolerance component, were performed as detailed in Condition Report (CR) # 129412.

Volumetric, surface and visual examinations listed in Table 3.1-1 that did not meet coverage requirements of Section XI of the ASME Code coverage of greater than 90 percent of the specified examination volume and area, are listed in Table 3.1-lA.

Supplemental examinations were performed on these components where practical to increase examination coverage. Relief requests will be generated and submitted within twelve (12) months of the end of the inspection interval as required by 10 CFR 50.55a(g)5(iii) and 50.55a(g)5(iv).

Volumetric and surface examinations were compared with the recorded results of previous examinations for all applicable Class 1, 2 and RISI components.

Pressure testing VT-2 exams not related to repair/replacement activities are documented in Table 3.1-2. Regarding Table 3.1-2, in the QlR16 90-Day Post Outage Letter (SVP-01-003 dated February 1, 2001) an error was noted following the submittal (CR # 129782). Specifically, the headings (from top to bottom) list the 2 nd Period followed by the 3 rd Period Inspections. The headings were inverted and should list (from top to bottom) the 3 rd Period Inspections followed by the 2 nd Period Inspections.

Exelon personnel performed functional tests on a total of thirteen snubbers for operability per the Technical Requirements Manual 10% sampling, Service Life Monitoring /Failure Trending Sample and ASME Section XI, Relief Request CR-09.

Eight (8) of the thirteen (13) snubbers were ASME Section XI Code designated. All snubbers were found acceptable for continued operation. Snubber testing activities are summarized in Table 3.1-3.

Table 3.1-4 summarizes the conditions noted during the examinations and corrective measures taken prior to returning the unit to service.

Generic Letter 88-01 Augmented examinations required by Generic Letter 88-01, "NRC Position on Intergranular Stress Corrosion Cracking (IGSCC) in BWR Austentic Stainless Steel Piping", were performed as listed in Table 3.2-1. BWRVIP-75 "BWR Vessel And Internals Project, Technical Basis For Revisions To Generic Letter 88-01 Inspection Schedules" (as modified by associated NRC Safety Evaluation dated May 14, 2002),

was implemented this outage. The original scope of the outage included 7 Category "C", 2 Category "D", 4 Category "E" and 1 Category "F" weld(s). The Category "F" Weld 02BS-F4 (partial weld overlay during Q1R16) was originally scheduled to be completed during Q1 R17. However, due to radiation exposure concerns, the weld was UT re-examined with increased volume (i.e. included the outer 50% of the original pipe wall) per EPRI recommendations for a limited use overlay examination. This was in accordance with the Change of Commitment letter (SVP-02-072) sent to the NRC on August 21, 2002. Engineering evaluation per IWB-3600 requirements determined the weld is acceptable for continued service. During Q 1R18, the re inspection, evaluation or repair of the weld will be addressed in accordance with the GL 88-01/B WRVIP-75 program.

3.3. Invessel Inspections (IVI)

To implement the requirements of the Boiling Water Reactor Vessel Internals Program, (BWRVIP) General Electric (GE) was contracted to perform the invessel inspections. The following components and assemblies were examined: Jet Pump Assemblies in accordance with the BWR Jet Pump Assembly Inspection and Flaw Evaluations Guidelines (BWRVIP-41), Core Spray Internal Piping in accordance with the BWR Core Spray Internals Inspection and Flaw Evaluation Guidelines (BWRVIP 18), Core Shroud Vertical Welds in accordance with the BWR Core Shroud Inspection and Flaw Inspection Guideline (BWRVIP-76), Top Guide in accordance with the BWR Top Guide Inspection and Flaw Evaluation Guidelines (BWRVIP-26), Reactor Pressure Vessel attachments associated with the Feedwater Spargers in accordance with the BWR Vessel ID Weld Inspection and Flaw Evaluation Guidelines (BWRVIP 48), Control Rod Guide Tubes, and Vessel Interior. An enhanced VT-2 visual inspection of the Standby Liquid (SBLC) Nozzle N10 was performed in accordance with the BWR Standby Liquid Control System Core Plate AP Inspection and Flaw Evaluation Guidelines (BWRVIP-27).

The Steam Dryer was also inspected during Q1R17. Existing flaws associated with the Core Spray piping welds 2P4d, 3P4d, and 4P4d welds were re-inspected. These existing flaws were first identified in 1998 during Q1R15 and have experienced some growth within expected crack growth limits. Additionally there were three recordable indications on Jet Pump 2 and 7 AD-3b diffuser welds. The evaluation concluded that the Core Spray and Jet Pump flaws are within acceptable limits and shall be re examined after two cycles of operation.

Generic Letter 88-01/BWRVIP-75 The Category "F" Weld 02BS-F4 (partial weld overlay during QlRI6) was originally scheduled to be completed during Q1R17. However, due to radiation exposure concerns, the weld was UT re-examined with increased volume. This was Pipe-Pipe (2 Seam Welds) in accordance with the Change of Commitment letter (SVP-02-072) sent to the NRC 02BS-F4 on August 21, 2002. Engineering evaluation per IWB-3600 requirements determined the weld is acceptable for continued service (refer to NRC SER TAC NO. MB6698 dated January 24, 2003). During Q1R18, the re-inspection, evaluation or repair of the weld will be addressed in accordance with the GL 88-01/BWRVIP 75 program.

In Vessel Inspections The growth of the existing Core Spray 2P4d, 3P4d, and 4P4d cracks along with the indications identified on the Jet Pumps 2 and 7 AD-3b welds have been evaluated as Core Spray 2P4d, 3P4d, and acceptable under CR #'s 132434 and 132335. The evaluation (GENE 0000-0010 4P4d welds 0362-LTI) considered the impact that the Core Spray and Jet Pump indications would have on the Quad Cities LOCA analysis and structural integrity. This Jet Pump #2 and #7 AD-3d evaluation determined that the amount of leakage assumed is more conservative than welds the current licensing basic LOCA PCT analysis and that long term cooling and structural margins are met. The Core Spray and Jet Pump indications identified during Q1R17 will be re-examined after two cycles 15 of 19
