Duke Energy Progress, LLC, is enclosing an American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code,Section XI, Form OAR-1 Owner's Activity Report, for the Brunswick Steam Electric Plant (BSEP), Unit No. 1. The report covers inspection activities performed during Brunswick Unit 1 Refueling Outage 22 (i.e., designated as B1 R22) for the third inspection period of the fourth inservice inspection interval.

Evaluation 409514 - through wall leak in 30" 1-SW-142-30-157 Service Water line per requirements of Code Case Service Water System Piping Category D-B / Item 02.10 N-513-3.

Spool 1-SW-142-30-157 NCR 2141499 was originated on 8/4/2017. The piping was replaced during 81 R22 outage under W/O 20191808 Task 01.

During Scheduled 81 R22 Ultrasonic Inspection a Circumferential ID connected flaw was detected. It was documented on BNP report VEN-18-028 and Framatome report BNPl-0M-2018-2. NCR-2190611 was generated and a full 1B21N4A-2-SW1-2 Augmented/ BWRVIP-75 Cat. D structural weld overlay was NOZZLE N4A EXTENSION TO R-A / Rl.11-2 performed per EC-411737 using SAFE END (DM)

Code Case N-740-2 under work order 20240085 Task 07.

The Component BWRVIP status has been changed from Category "D" to Category "E" and is scheduled to be re-examined during the next refueling outage

During Scheduled B1 R22 Inspection a Ultrasonic inside surface planar flaw was detected in the base metal adjacent to the weld. It was documented on BNP report VEN 034 and Framatome report BNPl-DM-Augmented / BWRVIP-75 Cat. D 1B21N4D-5-SW2-3 2018-9. NCR-2190996 was generated R-A / Rl.20-4 NOZZLE N40 SAFE END TO PIPE and an evaluation was performed by EXTENSION (OM)

Structural Integrity on report #

1800389.301 and documented under EC-411734 for continued service. The Component BWRVIP status has been changed from Category "D" to Category "F." The ASME required successive examinations are scheduled to be performed during the next three inspection periods.
