Duke Energy Progress, LLC (Duke Energy), is providing this Post Accident Monitoring (PAM)

Instrumentation Report in accordance with Brunswick Steam Electric Plant, Unit No. 2, Technical Specification (TS) 5.6.6, Post Accident Monitoring (PAM) Instrumentation Report. As required by TS 5.6.6, this report is being submitted within 14 days following entry into TS Limiting Condition for Operation (LCO) 3.3.3.1, Post Accident Monitoring (PAM)

Instrumentation, Condition B.

At 04:40 on May 3, 2022, TS LCO 3.3.3.1, Post Accident Monitoring (PAM) Instrumentation, Condition A, was entered as a result of losing position indication for Primary Containment Isolation Valve (PCIV) 2-B21-F058M, Jet Pump #16 Flow Low Pressure Excess Flow Check Valve (EFCV). TS LCO 3.3.3.1, Condition A, requires position indication be restored within 30 days. Initial troubleshooting determined that the loss of position indication is likely a result of an issue with the EFCVs internal reed switch. Additional troubleshooting and repair activities require Unit 2 be shutdown. As a result, position indication was not restored within 30 days per TS LCO 3.3.3.1, Condition A, and Condition B was entered requiring this report.

Duke Energy plans to complete repair activities to restore PCIV 2-B21-F058M position indication during the next Unit 2 refueling outage, scheduled for April 2023. Until position indication is restored, PCIV 2-B21-F058M position indication is determined by Jet Pump #16 flow indication which is checked once per day, at a minimum, per procedure 0PT-13.1, Reactor Recirculation Jet Pump Operability, to ensure conformance with the requirements of Unit 2 TS Surveillance Requirement 3.4.2.1 for jet pump operability.
