The information is as initially received without verification or evaluation, and is basically all that is known by Region IV staff in Arlington, Texas on this date.

Egj)).ty Licensee Emeroency Classification Washington Public Power Supply System Notification of Unusual Event Washington Nuclear 2 Alert Richland, Washington Site Area Emergency Dockets: 50-397 General Emergency X Not Applicable S t ect: REACTOR SCRAM DUE TO INADVERTENT TURBINE TRIP nt 12:29 p.m. (PST), on February 18, 1995, the WNP-2 reactor scrammed from 80 percent power due to a turbine trip. All safety systems functioned as designed.

One control rod experienced a position indication " data fault" and did not indicate fully inserted. The licensee by an alternate method subsequently verified that the rod was fully inserted. The turbine tripped and the reactor scrammed because an equipment operator (EO) had operated the turbine reset valve instead of the turbine test valve while conducting monthly turbine valve testing.

The E0 failed to adequately self-check that he had selected the correct valve.

An individual qualified as a shift suppert supervisor (SSS) was supervising the E0 and also operating equipment; however, the two had not established a routine for verifying that the proper equipment was selected prior to its operation. The two valves were labeled with metal tags and the surveillance procedure was clear on which valves were the test and reset valves.

The senior resident inspector responded to the site and observed the licensee's recovery from the scram and portions of the reactor cooldown.

The licensee proceeded to cold shutdown to correct an electrical separation concern with two containment isolation valves in the hydraulic lines for the reactor recirculatioa flow control valves and leakage of containmenf exhaust purge valves.

Reactor restart is planned for Tuesday, February 21, 1995.

The licensee notified the state 4 Washington. At approximately 2':24. p.m. ,

February 18, 1995, the licensee nw .fied the NRC operations center, who in-turn notified Region IV. -
