f APPENDIX A NOTICE OF VIOLATION Washington Public Power Supply System Docket No. 50-397 l Post Office Box 968 l Richland, WA 99352 License No. NPF-21 During an NRC inspection conducted on October 1 - November 5, 1987 violations of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedure for hRC Enforcement Actions," 10 CFR Part 2, I Appendix C (1987), the violations are listed below: Technical Specification 6.2.2.f states in part that:

"An individual should not be permitted to work more than .. 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> in any 48 hour5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> period. . . . Any deviation from the above shall be authorized by the Plant Manager or his deputy, or higher levels of management in accordance with established procedures..."

Contrary to the above, on June 16, 1987, a maintenance electrican worked 16.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> and on June 17, 1987, worked for 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> on safety related motor operated valves which exceeded 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> in a 48 hour5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> period. No authorization was granted to exceed this limi This is a Severity Level V Violation (Supplement 1) Technical Specification 6.5.1.5 states in part that:

"The auorum of the P0C necessary for the performance of the POC responsibility and authority provisions of these Technical Specifications shall consist of the Chairman or Vice Chairman and four members including alternates."

Contrary to the above, on September 4,1987, Plant Operations Committee meeting 87-35.1 was conducted with the Chairman and three member This is a Severity Level V Violation (Supplement 1)

Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit within thirty days of the date of this Notice, a written statement or explanation to the U. S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspection office at Washington Nuclear Project No. 2. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved; (3) corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
