The hood at the top of the damper has two bolts on the west side that are not properdy fastened.

The bolt to the north is bent while the bolt to the south is not tight and the hole is oversized (see photographs 4 and 5). Neither bolt has nuts that are fully engaged. The two bolts on the east side are properly fastened.

The hood is not a vital aspect for the operability of the fan ifthe connection were to fail during a seismic event. However, the bolts need to be properly installed.

2. Is the anchorage free of bent, broken, missing or loose hardware?At the louver on the roof, one of the bolts fastening the housing to the concrete pedestal has missing washers. However, this was judged not to be: a seismic concern since washers are not vital for the seismic performance of an anchor bolt. Washers are not required for installations using mild steel bolts and plates without oversized holes.Hole is not oversized based on observation.

Also for the housing of 1X41-COO9B, there are missing screws on the side as shown in the pictures.

This was judged not to be an issue since there is nothing for the screws to attach to at this location.

The holes are not a concern because the overhanging portion ofthe housing prevents the water intrusion.

Anchorage was compared against Dwg H-12613, Rev. 13 and SQUG package dated 7/22193. The housing at the roof is not a seismically qualified portion of the equipment.

There is insulation removed from a pipe attached to the equipment which was not replaced and stored imporperly (see photograph 3).This is not a seismic concem. CR 524535 has.been written to replace the insulation:

properly.There is a steel frame supporting a large insulated duct from the air handler that exhibits corrosion that is more than mild (see photograph 4). The corroded steel does not extend beyond the immediate surface.The remaining steel does not exhibit the same: corrosive appearance.

The weight of the duct and insulation is low with respect to the steel frame's capacity byjudgement.

There are 18 screws present.Therefore, this is judged not to be a seismic concem.CR523730 was initiated to replace these screws.SWEs judged this not to be a seismic concern due to the presence of multiple screws present Three bolts on the side (next to the staircase, underneath the edge) are missing. CR518594 was initiated to replace these bolts.S WEs judged this not to be a seismic concern due to the presence of more than 10 bolts (side and back).Bolts attaching the unit to the below steel members were found bent and holes were found without bolts.Per the SQUG evaluation and Drawing B-43459, Rev. D, the original anchor bolts have been replaced with four Welded steel tube members. The bolts were allowed to be removed orabandoned in place. The original anchorage was also identified as an IPEEE vulnerability.

It was verified that theý 4 steel tube.members are present and welded to below framing and the equipment.

Two loose bolts were found on the back of the equipment attaching the framing to the support. One of these bolts is near a welded portion, so it can not be tightened, this is okay due to the existing weld. This was judged not be a seismic concern due to the sheer number of existing bolts/scres present.Four long bolts underneath the cooler fan were observed.
