A nut is missing for the U bolt on the grounding bar for conduit MC5153 which,frames into top of panel. Contact is being made between. the conduit and grounding bar and the conduit. This is not a seismic concern.

Conduit at northeast comer of panel at top is not fastened to the grounding bar. This is not a seismic concern.

There is a cantilevered strut support which is unused (see photograph 3). It is very close to the. conduits coming out of panel, which could impact them during a seismic event. The conduit will protect the cables from being damaged and is not considered'a seismic concern. The spatial interaction is judged.to be seismically adequate.

A box of lamps is stored inside the panel-(see photograph 4) and there are two loose screws lying on the concrete surface (see photograph 5). CR 520076 has been wirtten to resolve these issues.

Orginally identified as an A-46 and IPEEE outlier due to potential impact to adjacent panels. DCR 90-015 detailed plates to be installed

,to connect the top of 1H21-P202 to the adjacent panels. The walkdown verified these plates were installed.

Hinge on door to panel'is detached from its connection (see photograph 3). The hinge is located at the middle of the door. There are two other hinges at the top and bottom that are, adequately secured. The two hinges is judged to be adequate to operate and support the door.

There are no relays attached to the door and the lightweight of the hinge will not cause any vibration to the panel toeffect relays. It is judged to be seismically adequate. CR 520072 has been written to reinstall the hinge.

Components inside panel were inspected visually in accordance with the direction provided in the FAQ related to opening panels.

The cover is small in mass and the screws currently securing it in place are sufficient to hold it in place. It is judged to be seismically adequate. CR 520191 has been written to install the missing screws.

PBI-523 is located inside panel 1C82-P001 and has two (2) missing screws (see photograph 5). CR 520191 has been written to install the missing screws. The component is light weightand the existing screws are sufficient to hold it in place.

There is a fire extinguisher stored approximately l'-O" from the panel.

The extinguisher is supported from a wall support and the seismic accelerations are not sufficient to cause the extinguisher to falL It is judged to be seismically adequate.

Components inside panel were inspected visually in accordance with the direction provided in the FAQ related to opening panels.

The actualflengths were longer than the lengths called out in the calculation. The additional capacity resulting from the longer length exceeds any potential reduction caused by poor installation. It is judged to be seismically adequate.
