The hood at the top of the damperhas two bolts on the west side that are not properdy fastened. The bolt to the north is bent while the bolt to the south is not tight and the hole is oversized (seephotographs4 and 5). Neither bolt has nuts that are fully engaged. The two bolts on the east side are properly fastened. The hood is not a vital aspect for the operabilityof the fan ifthe connection were to fail during a seismic event. However, the bolts need to be properly installed. CR 525153 has been written to install these bolts.

Anchorage I.. Is the anchorage configuration verification required (i.e., is the item one YE NE of the 50% of SWEL items requiring such verification)?

2. Is the anchorage free of bent, broken, missing or loose hardware? YJZ NE UO N/Al At the louver on the roof, one of the bolts fastening the housing to the concrete pedestal has missing washers. However, this was judged not to be: a seismic concern since washers are not vital for the seismic performance of an anchorbolt. Washers are not requiredfor installationsusing mild steel bolts and plates without oversized holes.

Hole is not oversized based on observation.

Also for the housing of 1X41-COO9B, there are missing screws on the side as shown in the pictures. This was judged not to be an issue since there is nothing for the screws to attach to at this location. The holes are not a concern because the overhangingportion ofthe housing prevents the water intrusion. See attachedpictures.

3. Isthe anchorage free of corrosion that is more than mild surface Y[ NiOF UO] N/AZl!

4. Is the anchorage free of visible cracks in the concrete near the anchors? YE NEI UO- N/AZ
5. Is the anchorage configuration consistent with plant documentation? YN NO UF- N/AU (Note: This question only applies if the item isone of the 50% for which an anchorage configuration verification is required.)

Anchorage was comparedagainstDwg H-12613, Rev. 13 and SQUG package dated 7/22193. The housing at the roof is not a seismically qualifiedportion of the equipment. SQUG package only included evaluation of bolts inside the intake.

There is insulation removed from a pipe attachedto the equipment which was not replaced and stored imporperly (see photograph 3).

This is not a seismic concem. CR 524535 has.been written to replace the insulation:properly.

There is a steel frame supporting a large insulatedduct from the air handlerthat exhibits corrosion that is more than mild (see photograph 4). The corrodedsteel does not extend beyond the immediate surface.

The remaining steel does not exhibit the same: corrosiveappearance.

The weight of the duct and insulation is low with respect to the steel frame's capacitybyjudgement. In its currentcondition, the support is judged to be seismically adequate.

Comments (Additional pages may be added as necessary)

SQUG walkdown identified equipment to have steel tabs welded to the existing steel frame for the equipment, which were to be anchored to the concrete roof This was observed to be installed during the walkdown.

Instructions for Completing Checklist This checklist may be used to document the results of the Seismic Walkdown of an item of equipment on the SWEL. The space below each of the following questions may be used to record the results ofjudgments and findings. Additional. space is provided at the end of this checklist for documenting other comments.

Anchorage

1. is the anchorage configuration verification required (i.e., is the item one Y 0 Nr-]

of the 50% of SWEL items requiring such verification)?

2. Is the anchorage free of bent, broken, missing or loose hardware? YZ N[ UE] N/AE]

Housing of the equipment has 4 screws missing in the back which attach the housing to the interiorframe. There are 18 screws present.

Therefore, this is judged not to be a seismic concem.CR523730 was initiatedto replace these screws.SWEs judged this not to be a seismic concern due to the presence of multiple screws present Three bolts on the side (next to the staircase, underneaththe edge) are missing. CR518594 was initiatedto replace these bolts.S WEs judged this not to be a seismic concern due to the presence of more than 10 bolts (sideand back).

Bolts attachingthe unit to the below steel members were found bent and holes were found without bolts.Perthe SQUG evaluation and DrawingB-43459, Rev. D, the originalanchorbolts have been replaced with four Welded steel tube members. The bolts were allowed to be removed orabandonedin place. The originalanchorage was also identified as an IPEEEvulnerability. It was verified that theý 4 steel tube.

members are present and welded to below framing and the equipment.

Two loose bolts were found on the back of the equipment attaching the framing to the support. One of these bolts is near a welded portion, so it can not be tightened, this is okay due to the existing weld. This was judged not be a seismic concern due to the sheer number of existing bolts/screspresent.

Fourlong bolts underneath the cooler fan were observed. However, they were qualified by the SQUG walkdown package dated 11/5/88.

The anchors were verified to match the information in the SQUG package (See Sheet 7).

There is mild surface corrosionon the channels on the back of the equipment However, this is judged not be a seismic issue since it is only mild surface corrosion.

2. Is the anchorage free of bent, broken, missing or loose hardware? YZ NO U[- N/AEl 1 of the 16 bolts on the single train anchorageis too short, does not go through the nut completely. The equipment is wide and heavy, therefore little or no tension load is anticipatedin the anchorbolts.

Remaining bolts on the same side of the equipment are adequate to take any shearif needed. Therefore, the SWEs judged that the overall equipment anchorageis seismically adequate.
