Side covers of the housing for 1X41-CO09B has 8 screws; however, 1X41-CO09C has 6 screws.Capacity of 6 screws is judged adequate for seismic loads. Therefore, this is not a seismic concern. The side covers are not part of the structure and are not in the load path. One of the bolts for a nearby conduit clamp is missing. Since the conduit is small and there are adjacent supports within lOft of the support, the load on the support is minimal. It was judged as not a seismic issue. A CR has been initiated to place the missing bolt (CR513284).See attached picture.8. Shear cracks are present on the top corner of the room door as shown-in the attached picture. One base plate for a conduit going to 120V A-C cabinet has an anchor close to this crack. Since the loads are small on the baseplate near the crack, the support is still seismically adequate.However, a CR has been initiated (CR513954).

Roof leakage problems have been observed due to stains on the walls. It does not affect the anchors of the equipment on the walls. Previous CRs have been written on this issue and actions were taken. to stop the leakage. The stains on the walls are from earlier occurrences.

The small gap is located near a support and a bend for the HVAC system that is located at the duct entry into the room, so the duct is very rugged at that location.

In addition, discussion with the System Engineer has determined that the duct is not safety related Therefore, damage to this duct or loss of duct pressure boundary is acceptable provided'the duct does not fall down and damage safety related equipment in the room. The SWEs judge that the impact will not be significant, so there is no potentially adverse seismic condition.

The monorail support in the northwest corner of the ceiling has a large crack with evidence of oil leakage from above approximately 1 "from the anchor bolt (CR 513992). The.location and size of the crack is sufficient to reduce the capacity of the anchor boit for the crane (CR 519998). There are several other cracks near conduit and equipment support anchors near the north and west walls that will reduce the anchor capacities.

The crack is a known condition and is being tracked by CR 2008100666.

2. Does anchorage of equipment in the area appear to be free of significant degraded conditions?

The leakage and stainingfrom the crack leaves doubt as to the existence of corrosion in. the bolts inside the concrete at these crack locations.

The bottom strut appeared to not be anchored to the wall, while the top two are visibly off the wall. However, upon further inspection, there is one (1) anchor fastening the bottom support, which is behind the conduit clamp. It is determined that the one anchor is sufficient to support the conduit as it is located in the center of the strut's length and is not eccentrically loaded by the conduit. The conduit turns horizontal and is supported approximately 8'-0" from the bend. It is judged to be seismically adequate.4. Does it appear that the area is free of potentially adverse seismic spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

1. Two bottle caps were discovered sitting loose on the floor next to a restrained bottle near 1L48-1R33 RWC.Pump Room IA (CR 515103). The caps were judge to not be a seismic issue since they are small and not. near any sensitive equipment that could be impacted.2. An abandoned.Handy-Cone was found sitting in the Hear-Here enclosure near the RB MG Set Room B at Column Line R13. The Handy-Cone was judged to not be a seismic issue since the object is light and not near any sensitive equipment that could be impacted.3. A rolling ladderwas found tied off but not chocked near Column Line 13.
