Since oxidation is only on the surface, the anchors are not degraded. Therefore, judged to not be a concern.

There is a Unistrutpipe clamp on the west wall missing a bolt. The pipe clamp is spreadopen, offering no restraintto the supportedpipe (CR 515734). There are intactpipe clamps above and below the open clamp, so the,unrestrainedspan of the conduit is small. The existing pipe clamps offer enough support to prevent significant movement during a seismic event.

There is mild surface oxidation on the flange bolts for RHR Pump 2E1 1-C002D. Since oxidation is only on the surface, the bolts are not degraded Therefore,judged to not be a concern.

SWE 's have determined that these Items arefarfrom seismic/safety related equipment, therefore they are seismically acceptable.

A knee-bracefor conduit supportnear Valve 2E41-F007 near the 2T41-BOOSB HPCIcooler has only one anchor bolt (CR 515489).

There is a hole in the base platefor the second bolt, but it appearsthat only one bolt wasever installedsince there is no hole in the concrete.

The knee-brace is one of apair of supportsfor a small.conduitnear the wall. The second knee brace is fairly large and.has all anchorbolts; so.

it is judged to have sufficient capacity to support the small conduit during a seismic event. This issue is addressedin CR 515489.

A "Hot Pipe" sign was found lying on the gratingon the 96' elevation of the Unit 2 HPCIRoom (on the Unit 2 HPCIRoom-CoolerPlatform) next to: the 2T41-BOOSA cooler but not attachedto anything.

The tag is light and not located near any sensitive equipment, so it isjudged that there is no potentially adverse seismic condition due to this loose tag (CR 519707).

The H01 test connection lead was found lying on top ofthe 2T41-BOO5A cooler. The lead is very light and is attachedby the connection cable to the body of the cooler. There is no delicate equipment in the vicinity of the lead, and the cooler is very rugged, so any impact will not be significant. Filter Train 2T41-DO07 and D008 have anchorbolts corrodedas shown in the attachedpictures. The corrosion appears to be severe enough to degrade the capacity ofthe some of the bolts. A CR has been initiated for this purpose (CR515661).

Support anchors on the floor for the exhaust fan (2T41-CO05B) ducts have cracks in the concrete at one of the anchorbolts. See attached pictures.A CR has been initiatedfor this purpose (CR515779).

Also, a loose nut was found in the room. It is judged not to be a seismic concern.

two holes drilled in it immediately adjacent to the pipe above the 2H21-POOl instrument rack that looks like it was intended as a pipe support.

The pipe has additionalsupports on either side of the missing support and is light, so it isjudged that the existing supports have adequate capacity to support the pipe. Therefore, there is no potentially adverse seismic condition. However, the missing support needs to be reinstalled.

There is a temporarypower cord strung along the building steel to the 2E11-COO2A RHR Pump. The bulk of the power cord is tied into a loop near a steel beam, which isjudged sufficient to prevent impact between.

Therefore, there is no potentially adverse seismic interaction.

The impact isjudged by the SWEs to be credible but not significant. Therefore, there is no potentiallyadverse seismic condition.

There are afew items (aplastic bag, a roll of caution tape and a small metalpiece) left loose in the contaminatedareaaroundthe 2E11-C002C RHR Pump. All of the items are small andlight and located awayfrom sensitive equipment, so arejudged to not create a potentially adverse seismic:condition.

The bearing plate is well supported and the bolts fully extend into the concrete below the grout. The grout is not considered in the bolt capacity.

Therefore, the concrete pads arejudged to be seismicallyadequate.

The pipe is supported Vertically butthe angle support and the U bolt in place to prevent the pipe from moving off the support laterally. Vertical seismic accelerationsare not greatenough for the bolt to be lifted out of place. It is judged to be seismically adequate. CR 523486 has been written to replace the missing/unsecurednuts.

The nozzles are small andrelatively light, so they are unlikely to cause any structuraldamage duringa seismic event.

However, due to theirlocation on the supportfor a high trip hazard panel, there is a concern that the nozzles could potentially cause a plant transient.

CONTROL Floor El. 112 Room, Area, Unit 2 Station:Battery 2A Instructions for Completing Checklist This checklist may be used to document the results of the Area Walk-By near one or more SWEL items. The space below each of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end of this checklist for documenting other comments.,

1. Does anchorage of equipment in the area appear to be free of YZ NEI U0J N/AU potentially adverse seismic conditions (if visible without necessarily opening cabinets)?
2. Does anchorage of equipment in the area appear to be free of significant Y1Z N[l U0 N/AEl degraded conditions?

There are three cover plate screws out of six screws and one cover plate screw out of six screws, respectively, missingfrom two junction boxes nearPenetration2Z43-H1064C (CR 524309). Both of the junction boxes have sufficient remainingscrews to secure the covers in place, so there is no potentially adverse seismic,condition.

3. Based on a visual inspection from the floor, do the cable/conduit YI NFI UI- N/AE1
raceways and HVAC ducting appear to be free, of potentially adverse seismic conditions (e.g., condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?

There are holes drilledin several ofthe supportsfor the HVAC running above the ladders in the batteryroom. The holes were drilledfor an earlieror alternateHVAC configuration.and arenot being used in the currentconfiguration. There are no missing bolts, so there is no potentially adverse seismic condition.

4. Does it appear that the area is free of potentially adverse seismic :spatial YN NEI UU] N/AU interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

i If the room in which the SWEL item is located is very large (e.g., Turbine Hall), the area selected should be described.

This selected area should be based on judgment, e.g., on the order of about 35 feet from the SWEL item.
