1 Commonwealth Edison Company Docket Nos. 50-254; 50-265 Quad Cities Station, Units 1 and 2 License Nos. DPR-29; DPR-30 l l

I During an NRC inspection conducted on September 24 through October 26,1996, two f violations of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG 1600, the  ;

violations are listed below:

Technical Specification 6.8.A required applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978, be implemented.

This regulatory guide included administrative procedures dealing with procedure adherence and maintenance procedures dealing with safety-related equipment.

a. Quad Cities Administrative Procedure (OCAP) 217-2, " Plant Posting Control," Revision 0, Step D.4.b, required in-plant. postings to be reviewed and signed by the shift engineer.
b. Procedure OCAP 1100-12, " Procedure Use and Adherence," Step D.4.d.(5) required: "Following procedural steps in sequence unless deviations are allowed by procedure."

Steps G 1 and 1.2 allowed portions of the procedure to be performed out of sequence at the discretion of the supervisors involved. i Contrary to the above,

a. A pressure-temperature limit graph from Procedure OCOS 0201-02, "Pomary -

System Boundary Thermal Limitations," Revision 5, was found posted in the '

control room on October 11,1996, which did not have the approval signature of a shift engineer,

b. On October 23,1996, procedure OCMMS 6600-03, Step I.12.h was performed before Steps 1.12.f and g. without authorization of.the supervisor involved.

This is a Severity Level IV violation (Supplement 1).

2. Appendix B of 10 CFR Part 50, Criterion ll, " Quality Assurance Program," requires the licensee's quality assurance program to provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained.

Contrary to the above, l l

The licensee failed to ensure maintenance technicians performing activities affecting quality on the shared standby diesel generator on October 22,1996, were trained i to assure suitable proficiency.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Commonwealth Edison is hereby required to submit a written statement of explanation to the U.S. NJclear Regulatory Commission, ATTN: Document Control Desk, Washington D.C. 20'a55 with a copy to the Regional Administrator, Region ll!, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response if an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the j license should not be modified, suspended, or revoked, or why such other action as may j be proper should not be taken. Where good cause is shown, consideration will be given to l extending the response time. I Because your response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary or safeguards information so that it can be p! aced in the PDR without redaction.
