NOTLCE OF V10LAT10H Commonwealth Edison Company Dor.ket Nos. 50-254; 50-265 Quad Cities Units 1 and 2 License Nos. OPR-29; DPR-30 During NRC inspections conducted on November 23 through 25, 1992, and on November 27, 1992., through January 11, 1993, violations of NRC requirements were identified. In accordance with the " General Statement'of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (Enforcement Policy) the violations are listed below:-

1. 10 CFR Part 50, Appendix B, C'riteria XVI requires, in part, that measures be established to ascure that conditions adverse to quality arte promptly identified and corrected. In the case of significant conditions adverse to. quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude ]

repetition. 1 Contrary to the above, on November 25, 1992, a condition adverse to quality was not promptly corrected. Specifically, the licensee failed to complete corrective actions for a- Notice of Violation issued on September 16, 1988, which involved a failure to take prompt corractive action to resolve a % diesel generator logic concern.

This is a .Saverity level IV violation (Supplement I). -

2. 10 CFR Part 50, Appendix B, Criteria V states, in part, that activities affecting. quality shall_ be prescribed by. documented instructions of a type appropriate to the circumstances and shall be accomplished _in ~

accordance with these instructions.

Contrary to the above, on December 14, 1992, an instrument mechanic installed a temporary alteration on a reactor vessel . level' transmitter wl.thout documented instructions authorizing tile activity.

This is a Sever'ity' Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Commonwealth Edison Company is hereby required to submit a written statement of explanation to the U. S.

Nuclear Regulatory Commission, ATTN: ' Document Control Desk, Washington D.C.

20555 with a copy to the U. S. Nuclear Regulatory Commission, Region III, 799 Roosevelt Road, Glen Ellyn, Illinois, 60137, and a copy to the NRC Resioent-Inspector ht the Quad Cities Nuclear St'ation with 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of- Violation" and should include for each violation: (1) the reason for the violation, ce if contested, the basis s for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

Notice of Violation 2 If an adequate reoly is not received within the time specified in this Notice, an order or a demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause_is shown, consideration will be given to extending the response time.
