BROWNS-FERRY CABLE SEPARATION ISSUE - MAY 11, 1989 MEETING (TAC 00493)

The TVA Projects Division' staff met with the Tennessee Valley Authority (TVA) '

on'May-11, 1989, at the NRC offices in Rockville, Maryland, to hear a presen-tation on the proposed resolution of the Browns Ferry cable separation issue.

The meeting was requested by TVA.'

The attendance roster is included at Enclosure 1 and the transcript of. the meeting is provided as Enclosure 2.

At a previous meeting with TVA on the subject of cable separation (March 14, 1989), the NRC staff had' identified its concern about the verification that needed to be performed by TVA to ensure that the installed electrical cable L

configuration matched the design configuration.

l. Subsequent to the meeting TVA performed a reverification of the data used in'the March 14, 1989 meeting. TVA-informed the NRC that some of the data provided in the March 14, 1989 meeting contained inaccuracies. The purpose L of this meeting was to provide TVA with an opportunity to present the results -

L of the reverification effort. .TVA indicated that .their evaluation of the data for ~ design significance involved detemining if there were discrepancies '

,1 between- the configuration of cables installed versus design configuration. t,

'When discrepancies were identified, they were evaluated for design signifi- -

h cance. . TVA defined design significance as not being in confonnance with O applicable codes, standards, or licensing requirements (i.e., FSAR design O criteria). TVA then reviewed the results to determine a confidence level and  ?

a quality level based upon the design significance acceptance criteria. TVA r

-indicated that this approach is analogous to NCIG-02, which is the visual weld acceptance criteria. .4 TVA indicated that for V1/V2 cables (instrumentation cables) the reverifica-tion reconfirmed that there are 64 cables in this sample population. Field inspection of these cables indicated no significant design discrepancies.

TVA-indicated that these are all Appendix R related cable and that they meet Appendix R requirements.

a For the V3 cables (control power cables), the reverification study provided a sample population of 266 cables. TVA's review of these cables indicated that there were three design significant discrepancies in the population of 266 cables. TVA indicated that based upon the three design significant discrepancies found, there was 95 percent confidence of a 97 percent quality level or greater for the total V3 population. TVA concluded that for the three design significant discrepancies there was zero safety significance.

Finally, TVA provided data on the V4/V5 (power cables). The reverification of V4/V5 provided 951 cables field verified. Out of the 951 cables, there were 8 design discrepancies identified. TVA indicated that they found zero safety significance from their review of V4/V5 cables and that this reverification of V4/V5 provided a 95 percent confidence of a 98 percent quality or greater for tne total V4/V5 population.

The staff requested that TVA document these findings in a submittal together with the EA/QA oversight review of the reverification process previously cited by TVA. TVA indicated that a submittal would be forthcoming concerning the resolution of the cable separation issue.
