REVISION

SUMMARY

This revision incorporated the following changes:

1. Added references to the Codes and Standards in effect during the Third inspection Interval.
2. Established qualifications for the personne performing the visualinspection and manually exercising as recommended in NUREG-1482.
3. Better defined the responsibilities of the Program Manager responsible for the Check Valve Disassembly Program.
4. Enhanced the Instructions Section to provide additional guidance regarding valve grouping, corrective actions to be taken, actions to be taken to extend the disassembly / inspection frequency, and sample expansion.

1.0 PURPOSE The purpose of this procedure is to provide the administrative controls for the Check Valve Disassembly Program to ensure compliance with the requirements of Generic Letter 89-04 and NUREG-1482.

2.0 REFERENCES

3.1 Full-Stroke Exercise The demonstration based on direct or indirect visual observation or other positive indication that the moving parts of a valve function satisfactorily through one complete cycle.

4.0 RESPONSIBILITIES 4.1 Supervisor - lST Program The Supervisor responsible for the overall implementation of the Inservice Testing H.cT) Program will ensure compliance to the requirements outlined in this proc Jure.

4.2 Program Manager The Check Valve Disassembly Program Manager is responsible for the following actions:

4.2.1 Maintenance and implementation of this program in accordance with this procedure; 4.2.2 Selection of valves to be disassembled and inspected; 4.2.3 Review of inspection results and deposition of any corrective action; l 4.2.4 Maintaining an inspection status of each valve; and 4.2.5 Trending and/or tracking the inspection results of each valve and the failure rate of each valve group.

5.0 INSTRUCTIONS 5.1 Test Requirements l

5.1.1 One valve from each valve group SHALL be disassembled, the valve intemals visually inspected for loose or corroded parts, and the disk manually exercised through at least one complete cycle in accordance with OPT-11.1.2.3. <

5.1.2 If possible, valves that are disassembled to verify forward !!ow (open) l capabilities should have a partial flow performed in accordance with an approved plant procedure after re-assembly and prior to returning the valve to OPERABLE status.

5.1.3 Valves that are disassembled to verify closure capabilities are not required to a partial flow performed after re-assembly.

5.1.4 Personnel performing the valve internals inspection and manually exercising the disk SHALL be qualified in accordance with 4 ANSI /ASME N45.2.6.  !

5.2 Valve Grouping Criteria 5.2.1 Valves may be grouped PROVIDED they are of the same design (i.e.,

manufacturer, size, model number, and material of construction) and have the same service conditions (including valve orientation).

1. Valve groups are listed in Attachment 1 or Attachment 2.

5.3 Disassembly / Inspection Frequency 5.3.1 Valve groups that contain only one valve SHALL be disassembled, the valve internals visually inspected for loose or corroded parts, and the disk manually exercised each refueling outage for the respective unit.

5.3.2 For valve groups that contain more than one valve, each valve in the valve group SHALL be disassembled, the valve intemals visually inspected for loose or corroded parts, and the disk manually exercised each refueling outage for the respective unit or at least once every 6 years.

t 5.3.3 A frequency, other than once each refueling outage or intental longer than once every six years, may be established provided an evaluation is PERFORMED to demonstrate that the proposed frequency will NOT

! effect plant safety and the applicable information listed below is I

provided and supported by plant data.

1. Each valve in the valve grouping has been disassembled and inspected.
2. The valve group's failure rate is less than 25%.
3. The inspection results document the condition of each valve and the valve's capability to be full stroked. -
4. A review of plant and industry experience is performed regarding the same type of valve used in similar service to ensure the proposed frequency is justified.

5. A review of the installation of each valve is performed for problematic location per EPRI's Applications Guidelines for Check Valves in Nuclear Power Plants.

5.3 Disags::mbly/ Inspection Frcqusncy 5.3.4 When inspection results for a valve group show a greater than 25%

failure rate, a review by the Program Manager is to be performed to determine if the group size should be decreased or whether more valves from the group should be disassembled during every refueling outage.

5.4 Valve Selection 5.4.1 One valve from each valve group SHALL be selected to be disassembled, inspected, and exercised per the frequency defined in Section 5.3.

5.4.2 A different valve from each group is required to be disassembled, inspected, and exercised at each successive frequency until the entire group has been tested.

5.4.3 Once the valves within a particular group have been disassembled and inspected, the same sequence of selections should be continued in subsequent refueling outages to the extent practical.

l 5.5 Corrective Action 5.5.1 If a disassembled valve is not capable of being full stroke exercised or there is binding or failure of valves internals, the valve SHALL be l declared INOPERABLE and repaired or replaced.

1. If the valve is repaired, the visual inspection and the manual exercising of the disk SHALL be re-performed prior to retuming the valve to sen/ ice.

! 5.6 Sample Expansion d

5.6.1 If a disassembled valve is not capable of being full stroke exercised or there is binding or failure of valves intemals, then the remaining valves in the same valve group SHALL be disassembled, inspected, and manually full stroke exercised during the same outage.

1. If the valve group contains valves in another unit (e J., Unit 1. Unit 2, Diesel Generator #1, etc.), the disassembly and inspection of the remaining valves in that unit may be deferred to the next available outage if an evaluation of the problem indicates that the condition would not impact continued operation of that unit.
