4 NOTICE OF VIOLATION Northern States Power Company Docket No. 50-282 Docket No. 50-306 As a result of the inspection conducted on April 13 through June 21, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violations were identified:

1. 10 CFR 50, Appendix B, Criterion II, states, in part, that the quality assurance program shall provide control over activities affecting the quality of identified structures, systems, and components, to an extent consistent with their importance to safety, and that activities affecting quality shall be accomplished under suitably controlled conditions.

Northern States Power Operational Quality Assurance Plan, Section 16.2, Maintenance Control, states, in part, that measures shall be established for the control of maintenance to safety-related systems and components to assure that repair and modification activities are performed in a manner consistent with its importance to safety and that upon completion of repairs and modifications the affected systems and components are inspected and tested to determine that the required work was performed satisfactorily.

Contrary to the above, the following examples of failure to adequately control modification activities were identified:

a. On May 29, 1986, and again on June 10, 1986, with Unit 1 at 4

full power, work adversely affecting plant operation (disabling of a rod position step counter) was performed with inadequate controls,

b. From the time of Unit I startup on April 10, 1986 until May 4, 1986, MV-32023, which has plant safety functions, was lef t without its limit switch cover in place following modification activities.

This is a Severity Level IV violation (Supplement I).

2. 10 CFR 50, Appendix B, Criterion XVII, Quality Assurance Records, states, in part, that sufficient records shall be maintained to furnish evidence of activities affecting quality and that the records shall include at least the Operating logs. Northern States Power Operational Quality Assurance Plan, Section 19.1 implements this requirement.

Contrary to the above, as of 7:00 a.m. on May 2, 1986 neither the Reactor Log nor the Operations Log contained entries related to the missing valve cover on MV-32023 which had been noted on May 1. (See Violation 1 above).

This is a Severity Level IV violation (Supplement I).

3. Technical Specification 6.5 states, in part, " detailed written procedures

. . . covering areas listed below shall be prepared and followed. . . .

C. maintenance and test. ...

Contrary to the above, on March 24, 1986, during testing of substation equipment, a relay technician failed to properly follow written procedures which resulted in loss of one source of offsite power and automatic starting of both emergency diesel generators.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
