50.499/78-01 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on January 10-13, i'

1978, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit Nos.

CPPR-128 and 129 as indicated below:

Failure to Follow Procedures During Concrete Placement 10 CFR Part 50, Appendix B, criterion V requires that activities affecting quality be prescribed by documented instructions, pro-l:

cedures, or drawings of a type appropriate to the circumstances and that these activities be accomplished in accordance with these instructions, procedures, or drawings.

Brown & Root construction procedure A040KPCCP-4, Revision 3 entitled, " Concrete Placement," states that when the ambient temperature is from 30 to 45 degrees Fahrenheit, the minimum

^f temperature of the concrete when delivered to the pour location

shall be 50 degrees Fahrenheit or higher.

Contrary to the above, on January 13, 1978, Concrete Test Load Ticket No. 03680 for concrete delivered and placed in placement IH2-WB indicated a concrete temperature of 46 degrees Fahrenheit when ambient temperature was below 45'F.

This is an infraction.

Brown & Root quality control procedure STP-QCP-4.4, dated April 1,1976, and entitled, " Concrete Inspection," defines character-e istics to be inspected during each concrete placement. Vibrators are to be inserted in the concrete between 5 and 15 seconds and i

at a spacing within the limits of Table 5.1.4 of American Concrete Institute Standard ACI 309-72,

  • Recommended Practice for Consolida-s tion of Concrete." This spacing is 2-1/2 feet for the vibrators

being used.

Contrary to the above, on January 10 and 13,1978, vibrator spacing on placements FH1-W202and FH2-W8 was observed to be as great as four feet.

This is an infraction.
