RESPONSE TO NRC INSPECTION FINDINGS S0lmi TEXAS PROJECT ELECTRIC GENERATING STATION DOCKET NO. 50-498 AND 50-499
Dear Mr. Seidle:
The following is our response to the infractions (Appendix A) identified in IE Inspection Report No. 50-498/78-01 and 50-499/78-01 dated January 27, 1978.
A) Actions to Correct Problems
- 1) Infraction - Cold Weather Concreting An investigation of factors in this incident revealed that Construction Procedure A040KPCCP-4, Revision 3, l
was violated primarily due to interpretation problems with ACI 306 as to whether cold weather conditions l
existed or not on the Bay of the infraction. ne interpretation problem has been solved by revising South Texas Project Specifications 2A010CS028-E,
" Concrete Construction" and 2A010CS001-F, " Concrete Supply" to clarify and define cold weather conditions.
I Further, Construction Procedure A040KPCCP-4 has been l
revised to reflect the specification requirements.

- 2) Infraction - Improper Vibration 1
Inspections of both placements after form removal by B6R QA/QC, B5R Construction and HLSP QA revealed no indications that proper consolidation had not been achieved.
Both FH1-W20 and FH2-WSA, W6 and W7 re-quired minor cosmetic repairs, but neither case was caused by improper vibration.
Quality Control Inspectors were retrained and recertified to correct the problem of their not detecting and correct-ing the improper vibration technique.
See paragraph E) 2) below for additional information.
B) Actions to Preclude Recurrence
- 1) Infraction - Co1d Weather Concreting, O. p,-

and are being complied with. HLSP OA surye Q1an_ce checklists hav.e_been.. revised..to. incorporate _these changes.
HLSP QA will continue to monitor B5R com-pliance with the applied requirements.
- 2) Infraction - Improper Vibration HL5P QA issued Stop Work Request #1 on Monday, January 16, 1978. This request stopped all concrete placement activities in the main plant area.
Corrective action and recurrence control were required for lifting the Stop Work Request.
B5R QA/QC held retraining for placement inspectors on January 20 and 21, 1978. The placement inspectors were recertified after successful completion of examinations, subject to HLSP approval.
B6R' Construction held a refresher course on concrete vibration for foremen on January 19, 1978.
HLSP QA has increased routine daily surveillance of placement activities and has added an item concerning vibrator operation to the appropriate surveillance check-list.
