Appendix A NOTICE OF VIOLATION Based on results of the NRC inspection conducted on March 21-23, 1978, it appears that certain of your activities were not conducted in full compliance with Appendix B to 10 CFR 50 as indicated below:
Failure to Follow Procedures for the Control of Special Processes 10 CFR Fart 50, Appendix B, Criterion IX requires that measures shall be established to assure special processes including welding are controlled.
Pittsburgh-Des Moines Steel Company (PDM) QA Manual, Revision 3, paragraph 4.3.6 requires that site QA personnel shall be responsi-ble for signing documentation of inspection steps as they are completed.
Contrary to the above:
On March 21, 1978, the IE inspector observed that, although Unit No. 2 containment liner weld seam No. 95 (identified on Drawing E6BE, Revision 3) had been only partially prepared for welding, Fabrication Check List, FCL 2TH-17.0 had been signed indicating that preparation was complete and the seam was ready for welding.
This is an infraction.
Note: This iten of noncompliance was corrected by the licensee prior to the IE inspector departing the site. The corrective action was verified by the IE inspector.
