DOCKET NO. 50-333 SPECIAL REPORT REGARDING NONFUNCTIONAL FIRE BARRIER PENETRATION SEAL

Dear Sir:

The New. York Power Authority (NYPA) submits this special report in accordance with Technical Specification-3.12.F.2 and 6.9.B.2. The report addresses our inability to restore a fire barrier seal to functional status within a seven day period.

Introduction:

During the 1992 plant outage, corrective repairs were required on the "A" Feodwater non-return valve, 34NRV-lllA.

This valve is physically located at the south end of.the main _ steam tunnel. The valve is-accessible either-through .

the Turbine Building steam tunnel or from the recirculation Motor / Generator Set Room directly above theLyalve via removable floor plugs. Floor plug access-is preferred for ALARA considerations. These floor _ plugs functionally provide fire barrier protection between-the Recirculation

_ Motor Generator Set Room and the Steam Tunnel as well-as-

_ providing a physical barrier between plant vital and non-vital areas.

Action Taken:

Maintenance repairs on 34NRV-111A'were' planned'and scheduled. 14 continuous security watch and a. roving fire watch were posted and one of:the floor plugs was removed at 1800 on December 12, 1992. These' watches remained in place until the floor plug was replaced at 1400 on December 21,.

Cause:

The scope of repair on 34NRV-111A was such that the work, post work testing and restoration could not be completed within a seven day period. The subject floor plug was

, replaced and restored to a functional status immediately following post-work testing acceptance and cleanup activities.
