Dear Sir:

The New York Power Authority (NYPA) submits this report in l accordance with Technical Specification 3.12.F.2 and 6.9.B.2. The report addresses our inability to restore a i fire barrier seal to functional status within a-seven day  !

period.

Introduction:

The Authority is installing a full area. fire suppression system in the east and west cable tunnels because existing l fire suppression systems in-these areas were determined to [

be inadequate due to. design and equipment problems. LER

  • 004-00 describes problems related to-the current fire i suppression system. The new system is.a wet pipe system with significantly increased flow rates. The new system required installation of a 12 inch drain line in addition to '

existing floor drains to drain excess' water in'the event of

  • a pipe break or accidental discharge. Nuclear Safety i Evaluation JAF-SC-92-094 was written to determine the impact  !

of this modification on nuclear safety and its effect on the plant fire protection program. As part of the modification  ;

installation, a'20' inch core bore was made in the safety  :

pump room floor on April 8, 1992. . Drain-piping was  !

installed, the penetration sealed and determined to be I operable on April 20, 1993. '

An installation procedure and work request were written for installation of the modification. Opening of the new penetration was planned and scheduled. A continuous fire 1 watch was posted and the plant fire brigade notified of work l to be performed. Proper planning and scheduling of the drain line installation kept the actual time that the penetration was open to a minimum.

j Cause:

The scope of work for making the penetration, installing the '

drain piping and resealing the penetration was such that all work could not be completed within a seven day period. This. l l penetration was open a total of 12 days during the-I modification installation.
