ABNORMAL 0CCURRENCE 76/01 Page 2 of 3 DESCRIPTION OF OCCURRENCE:

On January 5,1976, General Atomic Company stopped work on hydraulic oil system (91) relief valve vent line changes which had been requested by Public Service Company. The changes requested involved changing the relief valve discharge vent line from one location to another. This work which had been the subject of verbal discussions among Public Service Company management, General Atomic Company supervision, was part of a two week, around-the-clock effort to complete System 91 oil-side recirculation loop modifications.

This work was stopped because the documents authorizing the oil-side recircu-lation loop modifications only provided for disconnecting the common relief valve vent line as the first step of making the relief valve vent line changes, but did not include reconnection to the part of this system which had been in-dicated in modification discussions.

OCCURRENCE:

The plant configuration in existence for doing the oil-side recirculation loop work was identical to that permitting the relief valve vent line changes. No conditions in plant configuration differing from those approved,: existed. -Comple-tion of the ' relief valve vent line changes was a change of convenience, and would in no way have comprimised the safe operation of System 91, or had any

• adverse ef fect on the health or safety of the public.

CORRECT-IVE

' ACTION:

1. Written documentation authorizing the vent line change was completed prior to resumption of the work.

2. The work force has been instructed to have specific documents in hand covering the job to be done and to perform only those portions of the work described therein, even though the job to be done is separated into various steps.
