IDENTIFICATION OF OCCURRENCE:

The output breaker on "B" standby generator failed to close. This is identified as an abnormal occurrence per Section 2.1, paragraph (f) of the Fort St. Vrain Technical Specifications.

. DESCRIPTIO'N OF OCCURRENCE:

The weekly surveillance was being performed on the diesel generators. The operator had manually started the diesels on "B" standby generator and was

., synchronizing the diesels to the incoming voltage. He attempted to close the output breaker but it would not close. The second attempt also failed 4 to close the breake~r. The electrician was called and he determined the breaker was inoperable. ,

APPARENT CAUSE OF OCCURRENCE: Design Unusual Service Cond.

Procedure

, ANALYSIS OF OCCURRENCE:

The breaker was removed from the circuit and troubleshooting indicated the stored energy closing arm had come off of its shaft. These breakers use energy in a displaced spring to close. The keeper had come off the shaft, allowing the arm to slip off the shaft. This reduced the stored energy below the value needed to close the breaker. (See attached figure.) The keeper was bent and

coming off raises a doubt about initial assembly of the breaker. The breaker on LA standby generator has experienced the same service with no-problems.

CORRECTIVE ACTION: -

The stored energy arm was returned to its required position and a new keeper..

i installed. The breaker was then tested satisfactorily and returned to service.

A visual check of keeper will be added to the semi-annual preventive maintenance
