During routine surveillance testing, it was discovered that one of the heating ventilation and air conditioning (HVAC) dampers for the 480 volt switchgear i

room would not close. This results in operation under a degraded mode per-mitted by LCO 4.10.1 and is reportable per Fort St. Vrain Technical Specif1-cation AC 7.5.2(b)2.

EVENT DESCRIPTION: .

. . On September 25, 1978, during a normal shutdown maintenance period, operating i personnel performing routine surveillance testing, found one of the five (5) exhaust isolation dampers for the 480 volt switchgear room would not shut.

This is required to be operable per LCO 4.10.1, because of its function as a containment boundry during use of the Halon Fire Suppression System. The surveillance being performed was an annual one and during the surveillance interval, the plant had been at power. Therefore, this is reportable per Ft.

St. Vrain Technical Specification AC 7.5.2(b)2. .

See figure 1. ' A faulty o-ring in the damper actuator assembly jammed the actuating shaf t in place, holding the damper open.

CORRECTIVE ACTION: ,

( The faulty o-ring was replaced, the damper placed back in service and the surveillance was successfully coupleted.

! No further corrective action is anticipated or required.
