which is after most of my training was over. It is marked " Exhibit G" to this testimony. This was added to the site policy manual section " Hanger Process Control."

The letter references M916.

Q How did M916 come to your attention?

A It was not until approximately ten months after I began working at Hunter that M916 came to my attention. When drawing up the check list for 059-3, we saw this letter in the very back of the section for the subject of hanger process control. It appears on our checklist as question 10 out of 11 questions we asked, because we just happened to find it in the SIP. When we picked it to add to the checklist, we didn't know what the M916 looked like, nor why it was im-portant. It was really happenstance that we audited to that question, but it turned out to be a very crucial question, as well as a crucial document.

l Q Would it have been helpful to you if you had been familiar r

l with the M9167 A Yes, especially with respect to Audit 059-3. I think as an l

auditor I should have been familiar with every document that dealt with Quality Assurance. Otherwise, I could not have a thorough understanding of what I'm looking for in an audit.

" . , ,- Q Wh'ta problems did you uncover when you audited to this question?

A The question is related to the M916, for the as-built data requirement calls for the QCWI to make a reference to the M916's allowable tolerance if a support has been moved. We found, as indicated on pages 10 and 11, that IIC-QA-23, Exhibit G, was not being followed. Only one of the five hangers selected had QCWI acceptance of as-built data. Again, the QCWIs had not been trained to the M916.

Also, we found that the QCWIs did not even measure the location of the hanger to see whether it was within the M916 tolerances. As I said in my. affidavit, they didn't even have appropriate tools to make these measurements.

Q How was this item resolved?

A I left Hunter four and a half months after the audit, and Item 11, including Finding 7, discussed above, was still open when I left. Although technically closed on paper, I considered it open because no training had commenced by the time I left Hunter.

Q Was it unusual for an audit finding to stay open that long?

A Yes.
