Allegation No. 4-91-A-0018 This is in reference to our telephone conversation of April 2,1991, and our interview with you on April 19, 1991, in which you expressed general concerns related to you by other persons, both past and present employees at Comanche Peak Steam Electric Station.
These concerns are related to various aspects of operations at Comanche Peak.
An attachmer.t to this letter documents these concerns, as I understand them.
We have initiated actions to examine the facts and circumstances regarding the Plant labeling concern; however, the information provided for the remaining concerns lacks sufficient detail to allow us to initiate any actions at this l
time. We have asked that you provide us with more details or the telephone numbers of the individuals who provided this information to you, or arrange for them to contact us directly.
You have declined to do so.
In closing, let me assure you that every effort will be made to resolve these concerns. The U.S. Nuclear Regulatory Commission is responsible for i
protecting the public in the uses of nuclear facilities and materials. We engage in major efforts on a daily basis to fulfill that responsibility.

Unit 2, for the most part, is vastly mislabeled. This label problem may extend to Unit 1.

Documents were dumped on-site by administrative personnel in retaliation for an announced reduction in force.
3.
A woman, hired as a fire watch, was demoted for doing her job.
4.
Bare wires were run through a conduit between the units.
5.
Quality Control personnel sign off on work without actually inspecting the work.
6.
The protected area can be easily penetrated.
