01-31-82 TITLE OF THE EVENT: FLOW TRANSMITTER 1113 PLUGGED 1.

DESCRIPTION OF EVENT

On January 31, 1982, with unit 1 operating at 100% power, a loss of boric acid flow to the blender was observed while trying to makeup to the VCT.

This event is contrary to T.S.3.2.C 4 and is reportable as per T.S.6.6.2.b.(2).

PROBABLE CONSEOUENCES:

Rapid response to the flow blockage limited the loss of flow to a period of 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> and 10 minutes. Since during accident conditions, the BAST is isolated with Boric Acid being sup;,31ed by the RWST, the health and safety of the public were not affected.

CAUSE

This event has been determined to have been caused by a heat tracing failure of circuit 24A, panel 8 and circuit 24B, panel 9 (Unit 1 Boric Acid Filter and drain) combined with insulation damaged while cleaning the area. This loss of heat tracing combined with the loss of heat due to damaged insulation allowed the growth of boric acid crystals producing a slush which collected in the restricted area of the flow transmitter (FT-lll3).

4.

IMMEDIATE CORRECTIVE ACTION

Immediate corrective action consisted of removing the boric acid plug and verifying boric acid flow to the blender.

5.

SUBSEQUENT CORRECTIVE ACTION:

The defective heat tracing was replaced and tested within the time span specified by Technical Specifications. Damaged insulation in the area has been repaired or replaced.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

Since most loss of boric acid flow problems occur due to blockage at the flow transmitters, an Engineering Study has been recommended to determine if additional heating is required for flow transmitter.

7.

GENERIC IMPLICATIONS:

A task force has reviewed the total spectrum of the Heat Tracing System and a design change has been prepared as a result of the Task Force Study.
