Dear Mr. Ebneter:

This Special Report is being submitted per Technical Specifications, Section 6.9.2, due to Action 30 of " Radiation Monitoring Instrumentation,"

Specification 3.3.3.1 Table 3.3-6, Item d(i).

On March 22, 1990, the main plant vent, high range, radiation monitor (RM-A13) began to alarm spuriously. RM-A13 was declared inoperable and was removed from service at 1150 hours0.0133 days <br />0.319 hours <br />0.0019 weeks <br />4.37575e-4 months <br />.

Action 30 of Specification 3.3.3.1 allows RM-A13 to be inoperable for up to I

72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> before requiring a preplanned alternate method of monitoring to be

implemented. The alternate method of monitoring was initiated at 1130 hours0.0131 days <br />0.314 hours <br />0.00187 weeks <br />4.29965e-4 months <br /> l on March 25. It is important to note that Specification 3.3.3.1 is not applicable in Mode 5 and the plant entered Mode 5 at 2235 hours0.0259 days <br />0.621 hours <br />0.0037 weeks <br />8.504175e-4 months <br /> on March 25 l to enter the fifth refueling outage.

A Maintenance Work Request (MWR) that was initiated to troubleshoot and repair RM-A13 discovered a failed readout m( Ole as the cause for the spurious alarms. RM-A13 was repaired and returned to service on April 2.
