ATTACHMENT TO LER#82-010/03L-0 ITEM 10 DURING SURVEILLANCE TESTING, 2-CMO-420, "W" CCW HEAT EXCHANGER DISCHARGE, OPENED AS REQUIRED BUT WOULD NOT CLOSE FROM THE CONTROL ROOM. UPON INSPECTION, THE LIMITORQUE DRIVE SLEEVE WAS FOUND RUBBING ON THE HOUSING COVER. THE HOUSING COVER WAS MACHINED TO IMPROVE CLEARANCE AND REINSTALLED. THE VALVE TESTED SATISFACTORILY AND WAS RETURNED TO SERVICE.

ON FEBRUARY 3, 1982 THE VALVE AGAIN WOULD NOT CLOSE REMOTELY AND WAS REMOVED FROM SERVICE. THE DRIVE SLEEVE WAS AGAIN FOUND TO BE IN CONTACT WITH THE HOUSING COVER. THE COVER WAS ADJUSTED TO ALLOW CLEARANCE. THE VALVE TESTED SATISFACTORILY, WAS RETURNED TO SERVICE AND PREPARATIONS WERE BEGUN TO REPLACE THE VALVE.

CMO-420, A CENTERLINE VALVE, WAS REMOVED FROM SERVICE ON 2/05/82 AND REPLACED WITH A VALVE ?>ANUFACTURED BY PRATT. THE NEW VALVE TESTED SATISFACTORILY AND WAS PLACED IN SERVICE.
