The purpose of this submittal is to request a change to the name and address of the certificate holder on certain Certificates of Compliance (CoC). As the current certificate holder for the following packages, Columbiana Hi Tech LLC authorizes the transfer of the following CoC to TN Americas LLC:

This request for the name and address change supplements the application for renewal of the CoC for the Model No. UX-30 [4] and the application for changes to the package design for the Liqui-Rad (LR) Transport Unit Package [5]
