On February 17, 1992, at approximately 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />,. .it was determined that the plant stack gas flow transmitter, FT-1818, was not environmentally qua l Hied in accordance with Regulatory Guide (RG) I. 97 and 10 CFR 50.49. At the time of disc*ove*ry the plant was in cold shutdown for the 1992 refueling outage.

Flow transmitter FT -1818 is a required component by RG 1.97, Category 2, for stack gas vent flow per FSAR, Appendix 7C, Item E03A.

The root cause of this event is attributed to the lack of appropriate controls and design reviews during the development of the proposed list of instrumentation which assured compliance with the RG 1.97.

Corrective action for this event includes performing a temporary mod if i cat i ori on*the radioactive gaseous effluent monitor (RGEM) system to electronically simulate the flow conditions expected for normal operation of the HVAC system and. either replacing FT-1818 with an environmentally qualified flow transmitter or relocating FT-1818 to an area outside of the harsh environment. -

.On.February 17, 1992, at approximately 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />, it was determined that the pl~nt stack gas flow transmitter, FT-1818, [VL;FT] was not envtronmentally qualified in accordancewith Regulatory Guide (RG) 1.97 and 10 CFR 50.49. At the time of discovery the plant was in cold shutdown for the 1992 refueling outage. Fl ow transmitter FT -1818 is a required component by RG 1. 97, Category 2, for stack gas vent flow per FSAR, Appendix 7C, Item E03A. RG 1.97, Category 2 requires instrumentation to be environmentally qualified. Flow transmitter FT-1818 is located in thP component cooling room (Room 338) which is designated as a harsh environment, however, FT-1818 does not appear on the environmental qualification (EQ) list (E-48, Table 2.1).

  • This event is reportable to the NRC in accordance with 10CFR50.73(a)(2)(ii)(B) as a condition outside the design basis of the plant.

CAUSE OF THE EVENT The root tause of this event is attributed to'the lack of appropriate Controls and design reviews during the development of the proposed list of instrumentation which assured complianc~ with the RG 1.97.

This event did* not involve the failure of any equipment important to_saf~tY~

ANALYSIS OF THE EVENT NUREG 0737, Item II.F.1, required licensees to provide for continuous monitoring of plant effluents for radioactivity. The items requiring specific

-~ monitoring were iodin~, particulate and noble gases. Attachment 2 t~ Item lI I

II.F.1 of NUREG 0737 provided additional requirements for continuo~s monitoring capability. Among these was* a requirement for isokinetic sampling.

II In*order to obtain an isokinetic sample, the pl*nt stack effluent flow must be

! measured. This measurement is then fed back to the sample appara~us so that sample flow can be adjusted_ relative to stack flow to obtain a representative l sample of the stack effluent. At Palisades, FT-1818 provi~es. th~ isokinetic*

flow measurement capability. Therefore, flow transmitter FT-1818 is required to be environmentally qualified in accordance with RG 1.97, Category 2 and I

10 CFR 50.49 l A review of the documentation for this flow transmitter indicates the existing installation does not comply with the requirements of RG 1~97 for equipment I located in a harsh environment with respect to temperature and humidity.

These condition apply during a main steam line break (MSLB) accident scenario.

FT-1818 is located in the component cooling room which is classified as a harsh environment. Apparently, these environmental qualification requirements for fT-1818 were overlooked during the initial revie~ for Palisades compliance I with RG 1.97, which was conducted in 1983. Howeve~, .FT-1818 does appear in I

Appendix 7C of the Final Safety Analysis Report under Item E03A as requiring l environmental qualification. In 1986, FT-1818 was listed in the FSAR,

An alternate instrument, a differential pressure gauge {FI-1818) located outside the harsh environment of the component cooling room, is available to

~rovide the r~quired indication of plant stack effluent-flow .. This instrument will be relied upon to provide the required indication until FT-1818 can be

~ualified or relocated outside the harsh environment of the component cociling room.

CORRECTIVE ACTION Corrective action for this event includes:

  • 1. Perfo.rming a temporary modification {TM) on the radioactive gaseous effluent monitor {RGEM) system to electronically simulate the flow conditions expected for normal operation of the HVAC system. *This TM

  • ADDITIONAL INFORMATION

. There have been several previous events involving unqualified electrical equipment. The previous events are documented in CPCo corrective action documents D-PAL-90-014 and D-PAL-90-141. For ohe of these occurrences involvin~ the auxiliary feedwater circuits, the NRC issued a notice of violation {see NRC Inspection Report No. 90005).
