On January 22, 1998, with Unit 1 in Mode 5, personnel discovered screw heads in the ice condenser ice melt system trash filter. The sheared screws had apparently come from the ice condenser ice basket coupling rings, and the decision was made to conduct inspections of a sample of ice baskets in both units to determine their origin. The inspection identified a number of baskets which were missing more than the allowable numbers of screws at any one coupling ring. This was determined to constitute an unanalyzed condition, and an ENS notification was made on February 14, 1998 at 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br /> under 10CFR50.72(b)(2)(ii). This interim LER is submitted in accordance with 10CFR50.73(a)(2)(ii) as an unanalyzed condition found while the reactor was shutdown.

Briefly, each ice basket is approximately 12 inches across and 48 feet long. The original baskets were assembled in 12 foot lengths, using a sheet metal coupling ring. The ring is held in place by 12 screws arranged in 6 pairs. A recent evaluation concluded that a total of 8 screws would be sufficient to ensure that the sheet metal screw design load limits were not exceeded.

The investigation into this condition continues, as does inspection of the Unit 2 ice baskets. Other issues which impacted aspects of the ice condenser and its functions had been identified both before and after the identification of the missing screws. These issues are all being reviewed in parallel to ensure that the cumulative effect of these conditions are evaluated.

It is expected that an update to this interim LER will be submitted by March 31, 1998.
