On February 17, 1998, it was determined that the Unit 1 hydrogen recombiner surveillance requirement 4.6.4.2.b4 was not met on June 17, 1997. Surveillance requirement 4.6.4.2.b4 requires verification of the integrity of all heater electrical circuits associated with the electric hydrogen recombiners by performing a continuity and resistance to ground test immediately following a system functional test. Additionally, it has been determined that the recombiner surveillance procedure was not written in a manner to support the technical specification requirement to immediately perform resistance to ground test following the system functional test.

This event was reported on February 17, 1998 via ENS under the provisions of 10 CFR 50.72(a)(2)(l), as a condition found while shutdown that had it been found white the reactor was operating would have resulted in the nuclear power plant being in an unanalyzed condition.

Both hydrogen recombiners were declared inoperable pending evaluation of the event. The safety significance for this event is negligible as it has been determined that the hydrogen recombiner could have performed its intended safety function. This is an interim report pending further investigation and evaluation of the root cause. Corrective action will be performed based on the investigation findings. An updated report will be submitted by April 20, 1998.
