DURING THE INVESTIGATION OF OUT OF SPECIFICATION READING ON MFC140 (CHANNEL I STEAM FLOW) THE BISTABLES FOR THAT CHANNEL WERE PLACED IN THE TRIP MODE. DUE TO INCORRECT LABELING OF THE PANEL INDICATORS IT WAS THOUGHT'THAT'THE WRONG CHANNEL HAD BEEN PLACED IN THE TRIP MODE, SUBSEQUENTLY CHANNEL I WAS PLACED BACK TO NORMAL AND CHANNEL II WAS PLACED INTO TRIP. SEE ATTACHED SUPPLEMENT.

'UPPLEMENT TO LER ¹ 9-Q62 Q L-Q SUPPLEMENT TO CAUSE DESCRIPTION FURTHER INVESTIGATION REVEALED THAT CHANNEL I WAS INFACT THE INOPERABLE CHANNEL AND IT WAS PLACED BACK IN TRIP WITH CHANNEL II RETURNED TO NORMAL.

THE NAME PLATES FOR PANEL INDICATORS WERE CORRECTED AND THE NAME PLATES FOR THE OTHER STEAM FLOW CHANNELS, IN BOTH UNITS WERE VERIFIED.

A DESIGN CHANGE HAS BEEN INITIATED WHICH WILL IMPROVE THE CONSISTENCY OF AEP LABELING FOR THE STEAM FLOW CHANNELS AND ELIMINATE ANY CONFUSION IN LABELING.

THE ACTION REQUIREMENTS OF T.S. TABLES 3. 3-1 AND

3. 3-3 WERE FULFILLED.
