5.1 DESCRIPTION

This section describes those display instrumentation systems which provide timely information to the operator and allows him to adequately monitor conditions in the reactor, Reactor Coolant System (RCS),

containment, and safety related process systems throughout all operating conditions of the plant so that he may perform adequately all required manual actions important to plant safety.

Display Instrumentation is identified in Tables 7.5-1 and 7.5-3. Table 7.5-1 identifies the instrumentation which is provided for the monitoring of anticipated operational occurrences and of accident conditions.

Table 7.5-3 identifies the instrumentation which is provided for the monitoring of accident conditions as defined by NRC Regulatory Guide 1.97, Rev. 3.

The display instrumentation in Table 7.5-1 is tabulated in the following categories:

a)

Plant Process Display Instrumentation Information available to the operator for monitoring conditions in the reactor and related systems.

b)

Reactor Protective System (RPS) Monitoring Information available to the operator for monitoring the status of the Reactor Protective System (RPS) -

c)

Engineered Safety Features (ESF) System Monitoring Information available to the operator for monitoring the status of each ESF system.

d)

ESF Support Systems Instrumentation Information available to the operator for monitoring support systems required to function for the operation of ESF systems.

e)

Auxiliary Control Panel Instrumentation, see Section 7.4 f)

Control Element Assembly (CEA) Position Indication Information available to the operator for monitoring position of the control element assemblies.





WSES-FSAR-UNIT-3 7.5-2 Revision 8 (5/96) 7.5.1.1 Plant Process Display Instrumentation Table 7.5-1 lists the significant process instrumentation that is provided to inform the operator of the status of the reactor plant. This information, which is used for the start up, operation, and shutdown of the plant, is provided on the Reactor Turbine Generator Board (RTGB) and other Control Panels in the main control room. The information is provided in a form that is useful to the operator and may be indicated, recorded, or monitored. Alternate indication and control instrumentation is provided at local panels and the auxiliary control panel outside of the main control room to allow reactor shutdown and maintenance of the reactor in a safe condition during either hot shutdown or cold shutdown, should the main control room become inaccessible.

In addition to the hardwired display instrumentation listed in Table 7.5-1, there are 14 computer-driven CRT displays and two plasma displays on the Main Control Board. The plasma displays are driven by the Qualified Safety Parameter Display System (QSPDS) computers; one of the CRTs on the same panel is driven by the Radiation Monitor computers, while the remaining 13 CRTs are driven by the Plant Monitoring Computer (PMC). These displays can provide the main control room operators, at their request, with a continuous indication of the status of a specified set of parameters which can be used to define the safety status of the plant.



The plant monitoring computer is used for the Plant Safety Parameter Display System (SPDS) function (NUREG 0737 Supplement 1). Additional CRTs will be located in the Onsite Technical Support Center and the Emergency Offsite Facility. The Waterford 3 SPDS function is described in Appendix 7A.



7.5.1.2 Reactor Protective System Monitoring Although the RPS is automatic and does not require operator action, sufficient information is provided in the main control room to confirm that a Limiting Safety System Settings have been reached and a reactor trip has taken place. This information indicates process parameters that initiate reactor trip, pretrip, and bypass lights located in the PPS remote operators modules on the main control boards; audible alarms; CEA position information (Subsection 7.5.1.6); and trip switchgear circuit breaker position indication.

Operating bypass indication as described in Subsection 7.2.1.1.5 is provided on the remote modules, which are located on the main control board. Individual trip channel bypass indication is provided locally at the PPS cabinets, as well as on the remote modules on the main control board.

7.5.1.3 Engineered Safety Features Monitoring The Engineered Safety Features Actuation System (ESFAS) continuously monitors the system input parameters, and. performs actuation logic to initiate safeguards should these inputs reach their trip setpoints.

After the automatic actuation of the ESF systems, they continue to function properly without operator action. For example when the transfer of safety injection pump suction from the refueling water storage pool (RWSP) to the SIS sump is required, the recirculation actuation

WSES-FSAR-UNIT-3 7.5-3 signal automatically actuates this transfer. When the transfer is completed, the operator manually isolates the RWSP. (Subsection 7.3.1.1.2)

Even though the actuation of the ESF systems is automatic and does not require operator action, sufficient information is provided in the control room to allow monitoring of the operation of the ESF and related systems during normal operating and post accident conditions and to take any anticipatory action that may be desirable. This information consists of valve position indication, pump operating status, tank level indication, flow indication, RCS temperature and pressure, and indication of the process parameters that actuate ESF systems. In addition, four control modules, each associated with one channel of the PPS, are located on the control board to provide indication of the pretrip, trip, bypass, and operating bypass condition of each of the ESFAS input signals. Individual trip channel bypass indication is provided locally at the PPS, as well as on the remote modules on the RTGB.

7.5.1.4 ESF Support Systems Monitoring ESF Support Systems are those systems which are required to function for the operation of the ESF system. The instrumentation provided enables the operator to monitor important process variables and take appropriate action.

The Support Systems are as follows:

a)

Component Cooling Water System (CCWS) b)

Vital electrical distribution systems c)

Diesel Fuel Oil Storage and Transfer System d)

Heating, Ventilating and Air Conditioning (HVAC) for areas containing ESF Systems.

The CCWS, and HVAC Systems are operating during normal operation. On emergency signal those systems switch to an emergency mode of operation, and effectively support the operation of the ESF System. Proper environmental conditions are maintained in the main control room by the HVAC System in order to keep personnel safe and electronic equipment operational.

ESF Support System Instrumentation is located on various RTGB panels. Redundant safety channels are provided and identified as SA, SB and SAB. The operator observes selected parameters such as flow, pressure, temperature, level, and takes appropriate action to keep the systems in operating condition.

The power to ESF Support System Instrumentation is supplied by 120V uninterruptible AC system and 125V DC system. Safety related instrumentation for the systems outlined above is shown in Table 7.5-1.

7.5.1.5 Auxiliary Control Panel Instrumentation See Section 7.4.

WSES-FSAR-UNIT-3 7.5-4 Revision 12 (10/02) 7.5.1.6 Monitoring of CEA Position Indication Two diverse, independent CEA position indication systems provide CEA position information to the operator.

The systems are the Pulse Counting CEA Position Indication System and the Reed Switch CEA Position Indication System. These systems are described below. CEA position displays are located on the main control boards. These displays are described in Subsection 7.2.1.1.2.2. CEA position system block diagram is shown in Figure 7.2-4.

7.5.1.6.1 Pulse Counting CEA Position Indication System

The Pulse Counting CEA Position Indication System infers each CEA position by maintaining a record of the "raise" and "lower" control pulses sent to each magnetic jack control element drive mechanism (CEDM).

The pulse counting CEA position signal associated with each CEA is reset to a fixed value whenever the rod drop contact (located within the reed switch position transmitter housing) is closed (zero for a regular CEA).

This permits the pulse counting system to automatically reset the position to zero, whenever a reactor trip occurs or whenever a CEA is dropped into the core. This system is incorporated in the plant monitoring computer which feeds control board digital displays. One digital display provides individual CEA group information. A second digital display provides individual CEA position information. The position of each CEA is periodically printed out for a permanent record.

The Pulse Counting CEA Position Indication System provides position information to CEA related alarm programs and the Core Operating Limit Supervisory System (COLSS) contained in the plant computer.

Alarms are included in the system design to provide information to the operator required to maintain proper CEA control and to aid in the monitoring of CEA limits. The following alarms are provided by the pulse counting CEA Position Indication System:

a)

Power Dependent Insertion Limits Alarms An alarm is provided in the event CEA insertion exceeds predetermined limits required to maintain adequate shutdown margin and to insure CEA insertion consistent with the CEA ejection analysis. Further definition of the PDEL function is provided in Section 7.7.

b)

Pre-Power Dependent Insertion Limits Alarm This alarm is provided to advise the operator of an impending approach to PDILs.

c)

Out of Sequence Alarm An alarm is provided to alert the operator in the event the CEA groups are inserted or withdrawn in a sequence other than the predetermined acceptable sequence.

CEA Deviation Alarm An alarm is provided to alert the operator in the event the deviation in position between the highest and lowest CEA in any group exceeds a predetermined allowable deviation.

e)

Core Operating Limit Supervisory System (COLSS) Alarms The pulse counting CEA Position Indication System provides input data to COLSS. These data are used in the COLSS power distribution calculations, and alarms are initiated in the, event the affected COLSS limits are reached. The basis for the COLSS alarms and the use of the pulse count CEA position information is discussed in Section 7.7.

7.5.1.6.2 Reed Switch CEA Position Indication System The Reed Switch CEA Position Indication System utilizes a series of magnetically actuated reed switches (Reed Switch Position Transmitters (RSPT)) to provide signals representing CEA position. Two independent reed switch position transmitters are provided for each CEA. The RSPT provides an analog position indication signal and three physically separate discrate reed switch position signals. The analog position indication system utilizes a series of magnetically actuated reed switches spaced at 1-1/2 in. intervals along the RSPT assembly and arranged with precision resistors in a voltage divider network. The RSPT am mounted adjacent to the CEDM pressure housing, which contains the CEA extension shaft and actuating magnet. The analog output signal is proportional to the CEA position within the reactor core. The three discrete reed switch position signals are contact closure signals from three separately located reed switches. These signals are an upper electrical limit, a lower electrical limit and a rod drop contact.

CEA position information is provided to the core protection calculators (CPCS) directly and also to the CEA calculators. The CEA calculators display the position of each regulating and shutdown CEA to the operator in a bar chart format on a dedicated CRT on the RTGB. The operator has the capability to select either CEA calculator for display. In addition, a backup readout is provided that can be utilized to read the output of any CEA analog reed switch position signal. The backup readout is a digital meter on the CPCs operator's module, from which the operator can address any analog position signal for display on the digital meter. In addition to the displays, CEA deviation information is provided by the CEA calculators to the CPCs and a CEA deviation alarm. The CEA deviation alarm is provided to the plant annunciator system in the event a CEA calculator indicates that the difference between the highest and lowest CEA positions in a group exceeds a predetermined allowable deviation. The CEA deviation information is used in the CPCs determination of power distribution. The power distribution is then factored into the low DNBR and high local power density trip function. Pretrip alarms are initiated if the DNBR or local power density trip limits are approached. A pretrip alarm light is provided on the Plant Protection System control panel. Also, a pretrip alarm is provided to the plant annunciator system.

The three discrete CEA position switches provide signals (contact closure signals) to the Control Element Drive Mechanism Control System (CEDMCS) as shown in Figure 7.2-2. The signals are utilized to provide CEA limit indication on the RTGB and also to provide input

WSES-FSAR-UNIT-3 7.5-6 Revision 11-A (02/02) to the CEA control interlocks. Each of the three discrete reed switch contacts actuates an interface relay located within the CEDMCS. These relays provide contact signals for indication and control and, in the case of the rod drop switch, an additional contact signal is provided to the plant monitoring computer to reset the pulse counting system. The upper and lower electrical limits indication appears as two separate lights on the CEDMCS control panel mounted on the RTGB. The CEA drop indication appears on the core mimic display mounted on the main control board.

7.5.1.6.3 CEA Limit Lights Indication A light display is provided on the RTGB to indicate the fully withdrawn and fully inserted position of each CEA and provides indication of a dropped CEA.

7.5.1.7 Accident Monitoring Instrumentation The accident monitoring system is designed to monitor plant variables during and following an accident.

Indications of plant variables are required by the control room operating personnel during accident situations to (1) provide information required to permit the operator to take preplanned manual actions to accomplish safe plant shutdown; (2) determine whether the reactor trip, safety-feature systems, and manually initiated safety systems and other systems important to safety are performing their intended functions. (i.e.,

reactivity control, core cooling, maintaining reactor coolant system integrity, and maintaining containment integrity); and (3) provide information to the operators that will enable them to determine the potential for causing a gross breach of the barriers to radioactivity release (i.e., fuel cladding, reactor coolant pressure boundary, and containment) and to determine if a gross breach of a barrier has occurred. In addition to the above, indications of plant variables that provide information on operation of plant safety systems and other systems important to safety are required by the control room operating personnel during an accident to (1) furnish data regarding the operation of plant systems in order that the operator can make appropriate decisions as to their use and (2) provide information regarding the release of radioactive materials to allow for early indications of the need to initiate action necessary to protect the public and for an estimate of the magnitude of any impending threat. Instrumentation systems provided for the remote monitoring of accident conditions were qualified for operation in environmental and seismic conditions specified in Sections 3.10 and 3.11. Safety related instrumentation should continue to perform its safety function(s) and read within the required accuracy following, but not necessarily during, a safe shutdown earthquake. Table 7.5-3 shows that the instrumentation systems furnished in the design provide redundancy and appropriately wide range information of accident conditions.

The wide range of information furnished to the operator by the extensive instrumentation and control systems described in the table provide the long-term monitoring and surveillance capabilities of accident conditions.

¨ (DRN 00-25) 7.5.1.8 Bypassed and Inoperable Status Indication Waterford 3 does not have a Bypassed and Inoperable Status Indication System (BISIS), but has established other measures for indicating the bypassed or inoperable status of the protective system, systems actuated or controlled by the protection system, and auxiliary or supporting systems that must be operable for the protection system and the systems it actuates to perform their safety related functions.

Alternate means are available to comply with regulations implemented by Regulatory Guide 1.47.

 (DRN 00-25) 7.5.1.8.1 Reactor Protection System The reactor protection system monitors neutron power, hot and cold leg temperatures, pressurizer pressure, control element assembly position, reactor coolant pump speed, containment pressure, and steam generator level and pressure to detect conditions requiring a reactor trip and generate the reactor trip signal. The Reactor protection system energizes a reactor trip signal to de-energize the control element assemblies to drop into the core. The provisions for monitoring the reactor protection system are described in section 7.2.1.1.5, 7.5.1.2 and 7.5.2.2. These provisions include automatic indication in the control room to alert the operator when required protective actions have been bypassed or rendered inoperable. All bypasses are annunciated visually and audibly in the control room.

7.5.1.8.2 Engineered Safety Features Actuation System The engineered safety features actuation system monitors pressurizer pressure, containment pressure, containment area radiation, fuel handling building radiation, steam generator level and pressure, refueling water storage pool level, and emergency bus voltage to detect conditions requiring an actuation of the engineered safety features (ESF) and generate the required engineered safety feature actuation signal (ESFAS). Seven electrical signals (SIAS, CIAS, CSAS, MSIS, EFAS, RAS and FHA) are provided to actuate the appropriate ESF system, when required. The ESF system actuated by the ESFAS include:

safety injection, containment isolation, containment cooling, containment spray, main steam isolation, emergency feedwater, recirculating actuation, and ESF ventilation systems. The provisions for monitoring the engineered safety features actuation system are described in sections 7.2.1.1 5, 7.3.1.1, 7.5.1.3 and 7.5.2.3. These provisions include automatic indication in the control room to alert operators when required protective actions have been bypassed or rendered inoperable-Individual trip channel bypass indication is provided locally at the PPS, as well as on the remote modules on the RTGB control panel.

7.5.1.8.3 Systems and Support Systems Actuated by the Plant Protection System The systems and support systems actuated by the Plant Protection System and provisions to monitor bypassed or inoperable conditions of these systems are described in sections 7.3.1, and 7.3.2.

Additionally, safety related display instrumentation is provided as described in section 7.5.1.4, and Table 7.5-1 to monitor the status of each system and support system actuated by the Plant Protection System.

Improved regulatory guidance, design enhancements, and additional administrative controls have been implemented following issuance of Regulatory Guide 1.47 which assure the Operator has sufficient information to monitor the status of systems and support systems actuated by the Plant Protection System. NUREG-0700, NUREG-0737 and Regulatory Guide 1.97 were issued after Regulatory Guide 1.47. These documents address system operator configurations for human factors and the information needed by the operator to safely shutdown the facility. Enhancements were made in response to these documents.

A detailed review of the Control Room and Remote Shutdown Panel human factors was performed in response to NUREG-0700 and NUREG-0737. The enhancements recommended by this review were documented and implemented, as required, and the response to Regulatory Guide 1.97 was revised.

Design enhancements have been completed to improve the ability of the Operator to monitor the status of each system and support system actuated by the Plant Protection System. The control panels have been color coded by system and indicator scales have been human factored to allow the operator quick assessment of system status and process conditions. The non-safety related Plant Monitoring Computer provides the operator with real time information of system status on dedicated screens in the control room.

The Plant Monitoring computer functionality and capabilities are discussed in section 7.5a. The information provided by the Qualified Safety Parameters Display System, Safety Parameters Display

System, and Plant Monitoring Computer supplements the Plant Annunciator system, which provides system and major component alarms. Design changes to eliminate nuisance alarms from the control room have been implemented to assure Operator attention for alarms during plant operations.

Operational guidance has been enhanced to improve the ability of the Operator to monitor the status of each system and support system actuated by the Plant Protection System. Design changes have reduced the frequency of nuisance alarms, thereby minimizing the number of distractions and improving operator response. Administrative controls have been enhanced including measures to identify systems that are bypassed or inoperable, including: Equipment Out Of Service, Clearance Requests, Temporary Alteration Control, Independent Verification Program, Work Authorization Program, Condition Report Process, Control of Valves and Breakers, Alarm and Control Room Instrumentation Control. Conditions that render systems or support systems actuated by the Plant Protection System inoperable are subject to Technical Specifications action statements. Administrative controls to track entry into Technical Specifications action statements have been enhanced and are used to track the operability status of the systems and support systems actuated by the Plant Protection System.

The procedures for operating the facility coupled with human factoring of the instrumentation and controls in the control room and the design of the safety-related systems discussed in Section 7.5.1.8 comply with Criterion XIV, Inspection, Test and Operating Status of 10CFR5O Appendix B, Section 50.55a Codes and Standards of 10CFR50, and Section 4.13 of IEEE Standard 379-1971 Criteria for Nuclear Power Plant Protections Systems. A separate Bypassed and Inoperable Status Indication System (BISIS) is not required.

 (DRN 00-25) 7.5.1.9 Control Panels and Annunciators 7.5.1.9.1 Control Panels The RTGB is of the free standing benchboard type board with control switches primarily arranged on the lower bench portion, indicating and recording display instrumentation primarily on the lower vertical section, and annunciator windows primarily on the upper vertical section. The RTGB consists of six separate control panels and three plant monitoring computer CRT display panels and one Radiation Monitoring and Qualified Safety Parameter Display Panel located in the main control room and mounted adjacent to one another in a "U-shaped" arrangement. The panels are identified as follows:

In addition there is another safety related control panel in the main control room which is not a part of the U-shaped arrangement and is identified as CP33, hydrogen and cooling tower control panel.

In front of the main control board there is operators console CP9, which is not safety-related. The equipment located on operators console CP9 and on the main control board sections CP3, CP35, CP36 and the Shift Supervisors office is related to the Plant Monitoring Computer and although is helpful to the operator, is not safety-related.

Control switches and display instrumentation on the safety portion of the control panels are arranged functionally. Lights indicate valve positions or equipment status. Important system process parameters are displayed on indicators or recorders.

Safety-related display instrumentation which is a part of the PPS is divided into four redundant channels.

These redundant channels consist of mechanical and electrical devices and the circuitry involved in analog measurements to generate signals for the protective trips of the reactor. They are identified with the suffix SMA, SMB, SMC, and SMD.

All other safety-related instrumentation is either a part of ESF or support systems, and is divided into two redundant channels. These are identified with the suffix SA or SB. There is an additional safety-related channel which is identified with the suffix SAB. Equipment which belongs to this channel is used as a backup or a replacement for channel SA or SB.

All RTGB panels are classified seismic Category 1. The compliance of the instrumentation and control to the requirements of Regulatory Guide 1.75 is discussed in Section 8.3.

Figures 7.5-1 through 7.5-7 demonstrate the physical and electrical separation and identification of safety-related redundant channels.

7.5.1.9.2 Annunciators Waterford 3 has a total of 12 non-safety annunciator display units, each with 120 windows. and two safety-related annunciator display units, each with 45 windows in the main control room. Non-safety alarm contacts are wired separately to the non-safety annunciator logic and lamp cabinet. All alarm window displays are arranged functionally. Safety-related annunciators have two safety-related channels; one annunciator unit is a part of the SA channel and the other is a part of the SB channel. All safety-related redundant alarms are part of these units. The balance of the annunciator windows are utilized for other annunciation purposes. Alarm contacts for the safety-related annunciators are wired separately within each channel.

Audible alarms and visual annunciators alert the plant operator to the occurrence of malfunctions in equipment or systems, and signal changes in plant equipment status and operating modes. Alarms and status displays inform the operator of any deviations from normal operating conditions, such as the impending occurrence of automatic trips of the RPS and the status, malfunction, bypass or override condition of safety systems.

The arrangement of all annunciator windows is shown on figures listed in Table 1.7-1.

WSES-FSAR-UNIT-3 7.5-10 Revision 11 (05/01) 7.5.2 ANALYSIS 7.5.2.1 Plant Process Display Instrumentation Plant process display instrumentation gives the operator information to monitor conditions in the plant and to perform operations important to plant safety. In addition, the information allows the operator to crosscheck plant protection system measurement channels to ensure operational availability of these channels as discussed in Sections 7.2 and 7.3. The following design criteria were used in the selection of plant instrumentation:

a)

Provide continuous monitoring of process parameters required by the operator.

b)

Provide display information to the operator that is reliable, comprehensible, and timely.

c)

Provide instrumentation display that adequately monitors the parameter over the range required for various conditions.

d)

Provide a permanent record of those parameters for which trend information is useful.

e)

Provide four channels of indication for RPS and ESFAS process parameters to allow cross-checking of channels.

f)

Failure of a single indicator or one channel shall not adversely affect operators action.

Sufficient information is provided for the operator to accurately assess the conditions within the plant systems, to perform those appropriate actions in a timely manner, and to maintain the reactor systems within the conditions assumed by the safety analysis in Chapter 15.

7.5.2.2 Reactor Protective System Monitoring Sufficient information is provided for the operator to confirm that a trip has occurred and to determine which process parameter caused the trip. CEA insertion information after a trip can be determined by the operator from the CRT bar chart display information and CEA limit light indication. Indication of neutron flux levels in the reactor core, as well as other reactor and RCS information, is provided for the operator.

The following design criteria were used in the selection of information that is provided to the operator:

a)

System conditions requiring operator attention during routine plant operations and at the time of reactor trip are displayed on the RTGB.

b)

Annunciation at the control board of all operations performed at the PPS cabinet affecting the function of the system.

c)

Indication of all selected plant variables that are manually bypassed.

d)

Automatic or manual removal of a bypass shall be indicated.

7.5.2.3 Engineered Safety Features Monitoring

Information is provided so the operator can monitor the status of the ESF systems. The following design criteria were used in the selection of information that is provided to the operator:

a)

System conditions requiring operator attention or action during routine plant operations are displayed and/or controlled at the RTGB.

b)

All operations performed at the ESFAS cabinets that affect the functioning of the system are annunciated at the main control room.

c)

Indication is provided for all selected plant variables that are manually blocked or bypassed.

d)

Automatic or manual removal of block or bypasses shall be indicated.

e)

Information is provided to aid the operator in determining when manual actuation of an ESF system is required and to aid in confirming proper system operation after automatic initiation.

Input parameters that initiate actuation are indicated on the RTGB along with positive indications that pumps and valves have actuated and that flows have been established.

f)

Testing of ESFAS components can be accomplished on a scheduled basis during plant operation without adversely affecting the system operation.

Consistent with the above criteria the information which is provided for the operators use is shown in Table 7.5-1.

7.5.2.4 ESF Support System Monitoring Instrumentation for the ESF Support Systems is located on the RTGB and grouped functionally. Channels are identified the same way as the ESF System. Description of ESF Support System is discussed in Section 7.3. The following design criteria were used in selecting the process information for the operators use:

a)

Provide continuous monitoring of various process parameters that are essential for the proper operation of a particular system in a normal and an emergency mode of operation.

b)

Provide system conditions requiring an operators attention or actions.

c)

Provide redundancy of channels of instrumentation and control for reliable display and operation.

d)

Provide the capability for periodic testing of the equipment during normal operation.

e)

Provide display information which is accurate and timely.

Using these criteria the operator has sufficient instrumentation for his use to properly assess the situation during various modes of operation and take corrective action. The instrumentation is shown in Table 7.5-1.

7.5.2.5 Auxiliary Control Panel Instrumentation See Section 7.4.

WSES-FSAR-UNIT-3 7.5-12 Revision 14-B (06/06) 7.5.2.6 CEA Position Indication CEA position indication allows the operator to easily determine the position of all of the CEAs within the reactor core. The information is presented in a form that can be assessed by the operator to determine that the CEAs are in the required position, that a CEA has dropped into the core, or that the CEA positions are as required after a reactor trip.

The following design criteria were used in selection of the CEA position indication:

a)

Position readouts of all CEAs may be obtained.

b)

Continuous position indication of all CEAs is available.

c)

A means is provided to alert the operator to deviation of CEAs within a group.

d)

A permanent record may be made of the position of any or all CEAS.

e)

Separate "full-in" and "full-out" indications are provided for each CEA.

f)

Redundant and diverse means of indicating CEA position is provided.

7.5.2.7 Accident Monitoring Instrumentation Table 7.5-3 lists the variables committed to by Waterford 3 and recommended by Regulatory Guide 1.97, Revision 3. The table includes the Waterford 3 assigned category, range, environmental and seismic qualification, number of channels, power supply, type of control room display, availability on the Safety Parameter Display System (SPDS), quality assurance, and comments.

Those items which are not in complete compliance with Regulatory Guide 1.97 are discussed in the notes to Table 7.5-3.

7.5.2.7.1 Design Bases 7.5.2.7.1.1 Variable Type Definitions The variables identified in Table 7.5-3 are divided into five types in accordance with Regulatory Guide 1.97. The definition for each type of variable is as follows:

Type A - Those variables to be monitored that provide the primary information required to permit the Control Room operator to take specific manually controlled actions for which no automatic control is provided and that are required for safety systems to accomplish their safety functions for design basis accident events. Type A variables are plant specific and were selected based on a review of Emergency Operating Procedures to identify information essential for the direct accomplishment of specified safety functions. As a result of the review of Waterford 3 Emergency Operating Procedures and FSAR Chapter 15, the following variables were identified as Type A:

Primary information is information that is essential for the direct accomplishment of the specified safety functions; it does not include those variables that are associated with contingency actions that may also be identified in written procedures.

Type B - Those variables that provide information to indicate whether plant safety functions are being accomplished. Plant safety functions are (1) reactivity control, (2) core cooling, (3) maintaining reactor coolant system integrity, and (4) maintaining containment integrity (including radioactive effluent control).

Type C - Those variables that provide information to indicate the potential for breaching or the actual breach of the barriers to fission product releases. The barriers are (1) fuel cladding, (2) primary coolant pressure boundary, and (3) containment.

Type D - Those variables that provide information to indicate the operation of individual safety systems and other systems important to safety. These variables are to help the operator make appropriate decisions in using the individual systems important to safety in mitigating the consequences of an accident.

Type E - Those variables to be monitored as required for use in determining the magnitude of the release of radioactive materials and continually assessing such releases.

7.5.2.7.1.2 Evaluation Criteria As recommended by Regulatory Guide 1.97 each variable type was evaluated based on the importance to safety of the measurement of the specific variable.

Category 1:

provides the most stringent requirements and is intended for key variables. All Type A and some Type B, C, D and E variables are Category 1.

Category 2:

provides less stringent requirements and applies to instrumentation intended for indicating system operating status.

Category 3:

provides requirements that will ensure high quality off-the

-shelf instrumentation is obtained and applies to back up and diagnostic instrumentation. This category is also used when the state-of-the-art will not support requirements for higher qualified instrumentation.

The specific design and qualification criteria used to evaluate each variable, based on the category classification, are presented below:

Category I Environmental Qualification Currently the Environmental Qualification Program at Waterford 3 is in compliance with 10CFR50.49.

Regulatory Guide 1.97 Revision 3 Category 1 and Category 2 components located

WSES-FSAR-UNIT-3 7.5-14 Revision 11 (05/01) in harsh environments have been evaluated and are qualified within the scope of this program. The determination of qualification was based on having either actual environmental qualification documentation available or documentation on similar equipment available.

Seismic Qualification Currently installed instrumentation meets the requirements contained in Subsection 3.10. In general, original equipment was qualified with IEEE-344-1971, although in some cases, it was qualified to IEEE-344-1975. Instrumentation added as a result of the Regulatory Guide 1.97 Rev 3 guidelines for Category I equipment has been qualified to IEEE-344-1975.

Redundancy The location of the sensors and the points at which the sensing lines are connected to the process loop have been selected to provide physical separation of the channels to the maximum extent practicable, thereby precluding a situation in which a single event could cause failure of both channels, unless mentioned otherwise. Refer to Section 8.3 for further details for exception taken to the requirements for physical separation of redundant channels as defined in Regulatory Guide 1.75.

Power Supply Category I instruments are supplied with power from a Class 1E power supply. The Waterford 3 Class 1E power system is designed to meet the requirements of IEEE 279-1971, IEEE 308-1971, Regulatory Guides 1.6 and 1.32.

Quality Assurance The quality assurance program is described in the Quality Assurance Program Manual. These measures include appropriate requirements for design review, procurement, inspection, and testing which ensures that components are of a quality consistent with minimum maintenance requirements and low failure rates.

Display and Recording Continuous real-time display of at least one channel is provided in the Control Room. Recording of the instrument is provided for at least one of the redundant channels. Where it has been determined that direct and immediate trend or transient information is essential for operator information or action (Type A variable), a continuous dedicated recorder is provided with redundant backup recording and trending available on QSPDS and redundant dedicated indicators in the Control Room that can be utilized for trend information if necessary (one of the redundant dedicated indicators may be the continuous dedicated recorder listed above).

Service, Testing and Calibration Either channel may be tested, calibrated and/or repaired without detrimental effects on the other channel.

Performance of the surveillance instrumentation will be verified during reactor operation subject to the following:

1)

Testing will not adversely affect the safety or operability of the plant.

2)

Normal channel operation is considered an acceptable method of verifying surveillance instrumentation performance if channel operating parameters are similar to those anticipated following a LOCA.

3)

Where surveillance instrumentation performance cannot be verified under the conditions of one and two above, periodic testing is performed.

The loops can be checked from the sensor signal through the readout located in the main control room. Many of these sensors specified are also used in the PPS and, therefore, will be tested during PPS testing. For those sensors that are not part of the PPS, testing will be performed on a periodic basis.

Identification The channels are identified uniquely as indicated in Table 7.5-3. The channels are identified to distinguish between redundant channels of the same variable.

Category 2 Environmental Qualification - Same as Category I Seismic Qualification - No specific provision Redundancy - No specific provision Power Supply - Energized from highly reliable source (e.g., The non-Class 1E Vital).

Quality Assurance - Same as Category I Display and Recording - "On-demand" or continuous display is provided in the control room. No direct or immediate trend or transient information was determined to be essential for operator information or action.

Service, Testing and Calibration - Same as Category I Identification - No specific provision 7.5.2.9 Main Control Room Panels and Annunciators The main control room panels with annunciators provide the operator sufficient information concerning the plant condition. With the help of display instrumentation, equipment status indication, bypass and inoperable status indication, and annunciation, the operator can properly assess the situation during various modes of plant operation and take corrective or anticipatory action in order to maintain the plant in a safe condition.

7.5.2.11 Quality Assurance Quality Assurance procedures for handling, packaging, and shipping are consistent with the recommendations of Regulatory Guide 1.38 as discussed in the QA Program Manual.

7.5.2.12 Seismic and Environmental Qualification Applicable seismic and environmental qualification criteria, testing, and results are discussed in Sections 3.10 and 3.11, respectively.

For RCS pressure, Reg. Guide 1.97, Rev. 3 recommends a range of 0-4000 psig. Recorder and indicator have a range of 0-3000 psig. However, a range of 0-4000 psig is provided on the QSPDS which records on a 30 minute rolling buffer with the highest pressure value.

NOTE 3:

One containment isolation valve outside the containment is provided for certain lines penetrating the containment through Class C or Class D penetrations. (FSAR subsection 6.2.4.1.2 provides further details.) Dedicated indication for this valve is provided in the control room.

NOTE 4:

Two automatic valves are in series. NOTE 6:

CARS line through penetration #47 has valve CARMVAAA202B outside the containment and its actuator is powered from Train B. Line inside the containment branches into two, with isolation valves CARMVAAA201B and CARMVAAA200B on each branch. Actuators of valve CARMVAAA201B is powered from Train B and of valve CARMVAAA200B is powered from Train A. CARS is a back up to Hydrogen Recombiner System. CARS manually operated from main control room only during post accident, and after manually opening the locked closed valve CARMVAAA203B installed downstream of CARMVAAA202B. Valve CARMVAAA202B is also designed to close upon loss of power to operators. Hence, present configuration satisfies the intent of Reg. Guide 1.97, Rev. 3 to provide adequate indication of containment isolation.

NOTE 7:

Two valves on supply lines are installed, one on each side of penetration. Third valve on return line has a redundant check valve inside the containment. Valves located outside the containment are locked closed during Normal and Shutdown condition and are controlled administratively. Sensors of three automatic valves are wired in parallel to a common set of indication. The valves actuated through a common hand switch. This design and administrative controls provided adequate conservatisms to existing configuration to meet the intent of Reg. Guide 1.97, Rev. 3.

For Steam Generator Level, Reg. Guide 1.97, Rev. 3 identifies Steam Generator Level as a Type D Category 1 variable. It specifies a range of "From tube sheet to separators." Furthermore, within Table 1, Item 2 of Reg.

Guide 1.97, Rev. 3 it states the following with regard to redundancy, "Within each redundant division of a safety system, redundant monitoring channels are not needed except for steam generator level in two loop plants."

Waterford 3 meets the intent of these requirements using the following independent and diverse channels of instrumentation:

Two qualified channels of wide range steam generator level are utilized for each steam generator. Their range is 0-42.4 (509) feet which encompasses the distance from approximately 1 foot above the tube sheet and the separator.

2.

Four qualified channels of narrow range steam generator level are utilized for each steam generator. Their range encompasses just over 17.1 feet (205) encompassing the normal operating range of the steam generator.

The Plant Protection System provides protection against loss of primary heat sink by the Low Steam Generator Water Level Trip (LSGLT). Four narrow range instruments per SG are used for this trip signal initiation (Ins. Loops LT-1113A, B, C, D; LT-1123A, B, C, D for SG1 and 2 respectively). LSGLT set point corresponds to 5% of distance between the two narrow range liquid level pressure taps and is referenced from the lower tap. This set point corresponds to a water level of 31.3ft. above the tube sheet. The above mentioned instrument loops were also used for actuation of Emergency Feedwater Actuation System (EFAS). (Ref.

to W3-DBD-012 for further details) and High Steam Generator Level Trip (HSGLT). Whenever the level drops below or exceeds the narrow range level instruments, it will be monitored by either of the qualified independent channels of wide range SG level. In the extremely unlikely event that at least one wide range SG level indication was not available, the operator could determine level using diverse channels of indication. Using EFW flow coming into the SG, MS Flow going out of the SG and SG Pressure the operator can determine if SG level is increasing, decreasing or stabilized under/above the narrow range SG level.

Similarly, the operator could utilize SG Pressure, RCS Hot Leg temperature and RCS Cold Leg Temperature to determine SG level relative to the narrow range level indications. Such indications would be adequate until the SG level had returned to the range of the narrow range SG level instruments as directed by the EOPs.

For a loss of offsite power and failure of one diesel, the devices on the failed division only, are de-energized to conserve the battery in that division. This action extends battery power and provides additional time for trouble-shooting and restarting the failed diesel.
