ES-401 Record of Rejected K/As (RO) Form ES-401-4 Tier I Randomly Selected Reason for Rejection Group KIA Tier 1/ 025 Loss of Residual Heat This KIA requires the knowledge of the immediate actions for loss of RHR. At Group 1 Removal System (RHRS) Prairie Island, there are no immediate actions for a Joss of RHR. Immediate RO AK3.03: actions are designated in procedures by the steps having circles around them .

E-4, Core Cooling following Loss of RHR Flow, is the EOP for loss of residual heat removal and does not have any steps with circles around them. Randomly selected 025 AK3.01 as a replacement.

Tier 1/ 055 Loss of Offsite and This KIA Is Jess than 2.5. All KIA values for 055 EK2 are below 2.5. Randomly Group 1 Onsite Power (Station selected 055 EK3.02 as a replacement.

EK2.07:

Tier 1/ 065 Loss of Instrument Air Prairie Island does not use backup nitrogen to control valve positions in the event Group 1 AA2 .07 of a loss of instrument air. Randomly selected 065 AA2.06.

RO Tier 21 005 Residual Heat Removal This KIA requires the knowledge of the power supplies to the RCS pressure Group 1 System (RHRS) boundary motor-operated valves for the RHR system. Attempts to write a RO K2.03: question to meet this KIA resulted in a question that tested minutia or a question that had a level of difficulty rating of 1. Therefore, this KIA was rejected and replaced with 005 K2.01 .

Tier 21 008 Component Cooling This KIA is less than 2.5. All KIA values for 008 K5 are below 2.5. Randomly Group 1 Water System (CCWS) selected 008 A1 .04 as a replacement.

RO K5.05:

Tier 21 010 Pressurizer Pressure All three original KIAs in Category K5 in Tier 2 I Group 1 were rejected because Group 1 Control System (PZR PCS) of the KIA value being less than 2.5. Furthermore, all three of those KlAs could RO A3.01 : not be replaced with a KIA in Category K5 of their respective system because Category K5 of those systems have no KlAs above 2.5. Therefore, two systems within Tier 2 I Group 1 were randomly chosen to have there category changed to K5 to ensure requirement In Note 1 is met (i.e. Ensure that at least two topics from every applicable KIA category are sampled within each tier of the RO outline). Therefore , this KIA was rejected and replaced with 010 K5.02.

Tier 2/ 022 Containment Cooling This KIA is less than 2.5. All KIA values for 022 K5 are below 2.5. Randomly Group 1 System (CCS) selected 022 K2.01 as a replacement.

Tier 2/ 059 Main Feedwater (MFW) Prairie Island does not have turbine driven main feedwater pumps; they are all Group 1 System electric driven. Randomly selected 059 A2.03.

RO A2.07 Tier 21 061 Auxiliary I Emergency All three original K/As In Category K5 In Tier 2 I Group 1 were rejected because Group 1 Feedwater (AFW) System of the KIA value being less than 2.5. Furthermore, all three of those K/As could RO K3.01 : not be replaced with a KIA in Category K5 of their respective system because Category K5 of those systems have no KIAs above 2.5. Therefore , two systems within Tier 2 / Group 1 were randomly chosen to have there category changed to KS to ensure requirement In Note 1 Is met (I.e. Ensure that at least two topics from every applicable KIA category are sampled within each tier of the RO outline). Therefore. this KIA was rejected and replaced with 061 KS.03.

Tier 2/ 064 Emergency Diesel This KIA is less than 2.5. All KIA values for 064 K5 are below 2.5. Randomly Group 1 Generators (ED/G) selected 064 K6.07 as a replacement.

Tier 2/ 073 Process Radiation This KIA is less than 2.5. All KIA values for 073 K6 are below 2.5. Randomly Group 1 Monitoring (PRM) System selected 073 K4.01 as a replacement.

RO K6.03 Tier 2/ 078 Instrument Air System This KIA is less than 2.5. There is no other KIAs in 078 A2. Randomly selected Group 1 (IAS) 078 A3.01 as a replacement.

RO A2.01 Tier 21 103 Containment System This KIA is less than 2.5. Randomly selected 103 K1 .03 as a replacement.

Group 1 K1 .04:

RO Tier 2/ 055 Condenser Air Removal This KIA is less than 2.5. All KIA values for 055 K2 are below 2.5. Randomly Group 2 System (CARS) selected 055 A3.03 as a replacement.

RO K2.02 :

Tier 2/ 068 Liquid Radwaste This KIA is less than 2.5. All KIA values for 068 K3 are below 2.5. Randomly Group 2 System (LRS) selected 068 K1 .07 as a replacement.

RO K3.01 Tier 2/ 079 Station Air System This KIA is less than 2.5. All KIA values for 068 K3 are below 2.5. Randomly Group 2 (SAS) selected 068 K4.01 as a replacement.

ES-401 Record of Rejected K/As (SRO) Form ES-401-4 Tier I Randomly Selected Reason for Rejection Group KIA Tier 1/ 015/017 Reactor Coolant This KJA requires knowledge of the purpose and function of major system Group 1 Pump (RCP) Malfunctions components; which is RO ONLY knowledge. Therefore unable to prepare a SRO 2.1.28 question at the SRO license level for this KJA. Randomly selected 015/017 2.1.12 as a replacement.

Tier 1/ 022 Loss of Reactor Coolant Unable to prepare question at the SRO license level for this KJA. This KJA Group 1 Makeup requires the operator to determine whether a charging line leak exists. At Prairie SRO AA2.03 Island, Reactor Operators are required to diagnose leak locations on the RCS.

Also, at Prairie Island, 1C4 AOP1, is used to respond to a RCS leak. We attempted to develop a SRO question regarding this AOP to meet this KJA; however, it resulted in a question that only required the knowledge of entry conditions into other AOPs. This is RO ONLY knowledge. Randomly selected 022 A2.02 as a replacement.

Tier 1/ 025 Loss of Residual Heat Emergency Operating Procedure E-4, Core Cooling Following Loss of RHR Flow, Group 1 Removal System is used by the Shift Supervisor to respond to a Loss of RHR System. In E-4, the SRO 2.4.35 auxiliary operator initiates containment closure. While developing a question for this KJA, we researched EAL classification pertaining to this type of event. This research revealed a discrepancy with Prairie Island's EAL matrix with inappropriately referencing a TS Bases that no longer exists. The Exam Team determined this issue had to be entered into corrective action immediately instead of waiting until the exam project was complete. As such, this question and KJA is being rejected to prevent exam compromise. See CAP-01496615.

Tier 2/ 061 Auxiliary I Emergency Unable to prepare question at the SRO license level for this KJA. This KJA Group 1 Feedwater (AFW) requires the ability to predict the impacts of an air of MOV failure on the AFW and SRO A2 .07 use procedures to correct, control , or mitigate the consequences. On a loss of air, the TD AFW pump automatically starts. The MD AFW is not impacted on a loss of air. When the TD AFW Pump automatically starts on a loss of air, the only action is to secure the pump by closing the steam supply valves. This is RO knowledge and is an action that is taken in an alarm response procedure. There are MOVs in the AFW system; however, two of them are always valve open breaker open. The other four are always normally open, so that on a start signal to the AFW system, flow is automatically established to the steam generators. It was attempted to write a question on the fact that those valves must be closed after a non accident reactor trip to prevent overflowing the steam generators. If the MOVs were stuck open, the crew would dispatch an out-plant operator to locally close the motor valves using hand wheels. If the hand wheels did not work, then the crew would turn off the pumps to prevent overfilling the steam generators. These actions can be answered by solely knowing system knowledge of the AFW system. Therefore, th is is NOT SRO. Randomly selected 061 A2.09 as a replacement.

Tier 2/ 078 Instrument Air System Unable to prepare question at the SRO license level for this KJA. This KJA Group 1 (IAS) requires the ability to predict the impacts of a air dryer and filter malfunction and SRO A2.01 use procedures to correct, control, or mitigate the consequences. For an air dryer and filter malfunction at Prairie Island , the operators would enter 1C34 AOP1 , Loss of Instrument Air, and would trip the reactor if the air loss is significant enough. Entry conditions into AOPs are RO ONLY knowledge. Also, knowing when to trip the reactor is also RO ONLY knowledge. This KJA is the only KJA in A2 for Instrument Air System; therefore, the topic and KJA were rejected. Randomly selected 063 A2 .02 as a replacement.
