21.21 (d)(3)(i) DEFECTS AND NONCOMPLIANCE POTENTIAL PART 21 INVOLVING RTD TRANSMITTER The following is excerpted from a report submitted by Nutherm International, Inc.:

"The wire insulation in T2 transformer on the Moores Industry RTD transmitter Part Number: RBT/3W20-40/4-20MA/11I7AC/-EZ84.06-LNP-VTID [EX] was damaged during assembly. This damage reduced the insulation resistance and dielectric breakdown between the windings of the transformer. This resulted in early, catastrophic failure of the RTD transmitter.

"It has been determined that no visual inspection of the transformer or testing after the transformer is installed will discover this defect. This defect can only be found by performing testing on the transformer prior to installation."

This temperature transmitter has been sent to Entergy Operations - Fitzpatrick.

The following information is required per 100FR 21.21 (d) (4)

(i) Name and address of the individual or individuals informing the Commission.

501 South 11lth Street Mount Vernon, Illinois 62864 (ii) Identification of the facility, the activity, or the basic component supplied for such facility or such activity within the United States which fails to comply or contains a defect.

It has been identified that this temperature transmitter, RBT/3W20-40/4-20MA/1l17AC/-EZ84.06-

-LN-P-VTID [EX] as been sent to the following facility.

Entergy Operations - Fitzpatrick

('ill) Identification of the firm constructing the facility or supplying the basic component which fails to comply or contains a defect.

Nutherm International, Inc., 501 South 11th Street. Mount Vernon, Illinois 62864.

(iv) Nature of the defect or failure to comply and the safety hazard which is created or could be created by such defect or falture to comply.

The wire insulation in 12 trasformer on the Moores Industry RTD transmitter Part Number:

RBT/3W20-40/4-20MA/11l7AC/-EZ_4.06-LNP-VTD [EX] was damaged during assembly. This damage reduced the insulation resistance and dielectric breakdown between the windings of the transformer. This resulted in.early, catastrophic failure of the RTD trasmitter.

It has been determined that no visual inspection of the transformer or testing after the transformer is installed will discover this defect. This defect can only be found by performing testing on the transformer prior to installation.

Nutherm International, Inc. has concluded its review of all procurements of the RBT/3W20-40/4-2OMA/l117AC/-EZ84.06-LNP-VTD [EX] transmitters and have found two (2) units with this potential defect shipped to customers. The following facility was determined to be impacted by this condition:

Entergy Operations - Fitzpatrick Purchase Order Nutherm Traceability Number Serial No.

0649 13945-O1R1 1/01 2326051 10444073 13818-OlR1 1/01 2377572 (vii) The corrective action which has been, is being, or will be taken; the name of the individual or

• organization responsible for the action; and the length of time that has been or will be taken to complete the action.

Initial actions of determining the units aff~ete were completed 12/18/2015. The impacted customer was notified of this condition 12/18/2015. Corrective actions to prevent installation of damaged transformers consist of testing the transformer prior to installation. Nutherm obtained a unit with this testing performed. Unit was found to be free of the nonconforming condition and was tested at Nutherm International, Inc. The unit was found to be defect free and conforming to all specifications and requirements. Commercial Grade Dedication of this unit was completed 12/18/2015 and shipped to Fitzpaltick on 12/18/2015. The manufacturer has completed corrective actions to prevent any further nonconforming transformers being installed into any transmitters.

(viii) Any advice related to the defect or failure to comply about the facility, activity, or basic component that has been, is being, or will be given to purchasers or licensees.

Replacement of the units listed above is suggested.

(ix) In the case of an early site permit, the entities to whom an early site permit was transferred.
