Preventive Maintenance Mr. Martin opened the meeting by noting that the purpose of the meeting was to complete the discussions not concluded during the September 1,
1989, NRC/ANPP meeting.
Mr. Reilly then opened the discussion on preventive maintenance (PM) by describing recent licensee actions to assess the PM program.
The assessment identified weaknesses in management controls, and identified improvements to be made.in scheduling.
'The assessment effort also canvassed other utilities and identified good practices that could improve the PM program at Palo Verde.
Mr. Reilly stated that a scheduling task force was formed in June to develop and implement new PM scheduling practices.
A major change is that Palo Verde will use a rolling 12-week period to schedule PMs.
Under this program, PMs will be scheduled at the system level rather than the component level, and when a given system is removed from service once each 12 weeks, all the applicable system PMs will be grouped to be done at one time.
Mr. Martin observed that this practice should reduce the total integrated time that a given safety system is unavailable due to maintenance, and Mr. Reilly agreed.
Mr'Reilly described other improvement'ctions that are ongoing or already corn/I'eted, including program enhancements such as raising the level needed for authorizing the waiving of PMs to the Plant Manager.
Mr.
Martin questioned whether the licensee's stock of spare parts was adequate to support maintenance activities.
Licensee representatives described their spare parts inventory to be about average for the industry.
Mr. Martin observed that his experience was that a ready supply of the right spare parts is often reflected in the capacity factor of the plant.
Hr. Martin recommended that ANPP consider the spare parts area in conducting their assessment of maintenance.
Mr. Kirsch questioned whether ANPP had reviewed vendor recoanendations in assessing their PM program.
Mr. Reilly replied that about 5000 PMs were reviewed prior to the Unit 2 restart in late June, and that the results were considered to be good.
Reviews are continuing and are focusing in the electrical area, which is considered to have problems.
All 25,000 PMs will be reviewed over the next several years.
Mr. Reilly then discussed long term actions planned in the PM area.
A major goal is to establish a maintenance basis file, such that the reason for any given maintenance task or its frequency can be easily determined.
Mr. Conway closed the discussion in this area by assuring that all improvement programs described by his staff will be or are presently funded; however, Mr. Conway hesitated to carrnit to completion dates until a progress trend can be established and evaluate I
Nr. Reilly outlined the actions taken since June 1989, to improve performance fn the System Engineer area.
Actions taken have included management discussions with various groups from the Standards and Technical Support organization to discuss ANPP expectations regarding accountability and responsibility; demanding higher standards of excellence in work performance; clarifying work standards during annual performance reviews; the formation of a stronger non-conformance report system; the addition of work space for System Engineers; and trafning of engineers, including component specific training.
Mr. Reilly considered the results to date to be hard to quantify in that there has been some progress, however, there have also been failures.
Nr. Reilly continued that an additional important aspect that remains to be fully addressed is management control of System Engineering work assignments.
Mr. Reilly explained that System Engineers receive work.from many sources, which has caused frustration for both management and the engineers.
Steps are being taken to ensure management has a firm knowledge of and control over work assignments.
Additionally, Mr. Reilly indicated that ANPP needs to identify what tasks a System Engineer must do, and what tasks would"be better per'formed 'by 'another group.
Mr. Martin agreed that attempts to hold System Engineers accountable for their systems 'is meaningless if they. are overloaded with a large amount
'f non-essential tasks to'complete.
This may mean that more people are needed.
Mr. Martin continued that what is lacking is a sense of ownership of their systems by the engineers.
Management's task is to ensure that the System Engineers focus on the hard problems, and that they not be distracted by numerous non-essential and easy to accomplish

tasks.
Nr. Martin concluded by stating that ultimately, the System Engineers should be concerned about what hasn't gone wrong with their systems yet, and how they can identify potential failures prior to the problems becoming self-revealing.
Mr. Reilly agreed.
Cor orate En ineerin Involvement Nr. Allen then presented a discussion of actions taken to increase corporate engineering involvement in site activities and problems, as outlined in the licensee's enclosed presentation package.
The major points made by Mr. Allen included making clear to corporate engineering that they are expected to get involved in site problems and to be a full partner in plant operations, and that strong comunication links need to be established between site organizations and corporate engineering.
Nr. Allen described a program being implemented in October wherein cor'porate engineers will participate in system walkdowns..
Mr. Martin offered that in commencing this program, ANPP sho'uld consider at first not holding the engineers accountable for problems found until a baseline is established.
However, after the initial walkdnwns are completed, the engineers should be he'ld accountable for previously unidentified problems which then arise.
Additionally, Mr. Martin encouraged ANPP to hold personnel accountable for issues which are known, but are not effectively addressed.

Mr. Allen then went on to describe the re-establishment of the resident engineering function at the site, with about 10 personnel presently assigned.
Mr. Allen emphasized that this group is intended to be prob1em solvers, not just paperwork expediters.
Mr. Allen outlined engineering training that has been provided to corporate engineers and other site groups.
Hr. Allen stated that a number of lessons had been learned from their recent efforts to critically review design changes after installation of the design changes is complete.
Nr. Martin stated his support for such reviews. noting that people need to receive feedback on how well they are doing their work.
Mr. Allen concluded the discussion in this area by describing ANPP's participation in a joint utility engineering task force and by outlining what is expected of engineering personnel while they are at the site.

Qualit Assurance Hr. Ballard opened the discussion in this area by describing actions,to increase the capability of the Quality Assurance (OA) organization.
ANPI? management is considering rotating personnel from other departments into'QA, but this has not yet occur red. Expertise in areas such as operations and health physics have been obtained through contractors.
QA has increased their capabilities by hiring 6 degreed professional engineers and by sharing resources with other Region V utilities.
Mr. Martin stated that the issue is that OA does not appear to be fully competent to scrutinize areas such as operations and maintenance.
Nr. Ballard replied that his intentions are to gain the necessary talent in the short term through contractors and plans to hire personnel.
He indicated that by the end of 1990, a rotation program will be in place at the site.
Mr. Hartin reiterated the importance of manning OA with personnel competent in areas such as operations and questioned what presently prevented a Senior Reactor Operator from being assigned to OA.
Hr. Ballard replied that the present plant status required licensed operators to remain in operations.
Mr. Martin stated his skepticism of the ANPP plans.
Mr. Ballard then discussed actions taken to improve OA involvement in plant activities.
These included requiring personnel to be more operationally oriented; strengthening the NCR program; establishing an automatic escalation program for significant OA findings; establishing new and more easily used trending programs; providing'ore reports to management on where problems exist; and significantly increasing the amount of activities being monitored.
Nr. Martin questioned what were the four biggest issues recently identified by OA.
Nr. Ballard replied that those issues were problems with comnitment tracking; radiological control issues with cooling tower sludge; training and qualification problems; and problems with the Engineering Evaluation Request process.
Mr. Ballard then described ways in which ANPP management has utilized QA, including verifying the completion of restart items at both Units
and 3, conducting an internal maintenance team inspection, and conducting special reviews as requested by senior managemen l
Mr. Martin closed the discussion in the OA area by stating that much of what was,presented was positive, however, much was still very programoatic.
Mr. Martin again emphasized the importance of having strong capable personnel assigned to OA, if the adequacy of programs is truly to be critically assessed.
Mr. Martin felt that ANPP management had not yet fully addressed the issue of OA staffing.
He observed that QA also needs to. be held accountable, which includes answering for significant problems that are self-revealing rather than first being identified by OA.
Mr.
Martin offered as an example, the lack of management presence in the units, which should have been identified first by QA.
Mr. Martin closed by observing-that there needs to be very high expectations for QA during the future restart of the units.

Coneercial Grade Dedication Pro ram Mr. Huey opened this area by describing recent concerns identified by NRC inspections with ANPP's commercial grade dedication program.
Mr. Huey stated that the NRC is interested in ANPP assessing the NRC concerns in a timely manner, evaluating if a generic problem exists, determining a
root cause, as appropriate, and justifying continued operation, if need be; Mr. Caudill then described the ANPP efforts to maintain their prog'ram current with 'industry improvement's in this area and to assess the NRC concerns.
Mr. Ballard offered that this area is still under active review.
Mr. Martin and Mr. Zimnerman indicated that a detailed discussion wasn't intended for this meeting, however, this area typically doesn't receive much management attention, but can quickly cause major problems.
Therefore, the NRC intends to focus increased future attention in the areas of coainercial grade dedication and vendor surveys.
7; Cotanitment Trackin 8.
Mr. Rogers described the ANPP system to track comoitments made to the NRC, as outlined in the'nclosed licensee presentation package.
It was noted that some'oomitments are backlogged, primarily due to manpower.
Mr. Martin observed that ANPP may need to reassess/reprioritize past commitments, and that ANPP should not blindly pursue commitments that no longer make sense.
He offered that the NRC is open to change in comnitments where appropriate.

Mr. Martin closed the meeting by stating that he was encouraged in the area of System Engineers in that it appears that ANPP is focusing on the issues of accountability and clearly defining work expectations.
