REPORT OF MEETING WITH ANPP MANAGEMENT This refers to a meeting held with Mr. 0.

De Michele, yourself, Mr.

B. Karner, Mr. J.
G.
Haynes, and other members of your staff, with Dr.
T. Murley, Director of the NRC Office of Nuclear Reactor Regulation, myself, and other members of the NRC staff, at the Palo Verde Nuclear Generating Station, on June 5, 1989.
The subjects discussed 'are summarized in Meeting Report Nos.
50-528/89-29, 50-529/89-29, and 50-530/89-29, enclosed herewith.
As we have previously communicated to you, the circumstances surrounding the events at Units 1 and 3 in early March, 1989, are of significant concern to the NRC.
We were encouraged by this meeting in that the presentations made by.
yourself and your staff indicate a recognition of the seriousness of those events and the need for change in the way Palo =Verde is managed and operated.
We will closely follow your efforts to implement the actions you described in the meeting and in recent correspondence to the NRC.
Specific areas, of concern discussed during this meeting which warrant your prompt attention are summarized as follows:
The System Engineer program has been slow to develop.
The System Engineers themselves have appeared to lack an understanding of what is expected of them and apparently do not feel responsible and accountable for their system's performance.
In general, root cause evaluations have often been limited in scope and have not reviewed problems and events to the degree necessary to thoroughly correct the cause.
Specifically with regard to your root cause evaluations of the Atmospheric Dump Valve (ADV) failures and the problems encountered with the Compressed Gas System (CGS),
we concluded, and so stated to you during our meeting, that your evaluations did not determine why the various technical problems were not adequately addressed prior to the March, 1989 events, although the problems-were known to your staff.
In addition, you did not appear to have clearly resolved all of the technical issues at the time of the meeting.
As discussed*during the meeting, we understand that you will complete a more in-depth evaluation of the manner in which the ADV and CGS problems were previously handled:
We request that you provide this evaluation to the NRC within 60 days of the date of this letter.

"UL - >>ass The guallty Assurance and Oversight organizations have not been a visible force in the operation of Palo Verde.
Senior Management has not demanded that these groups be critical and probing in their reviews, such that major problem areas will be identified prior to becoming self-revealing.
ANPP Management should take the measures necessary to ensure that managers and supervisors are responsive to employee concerns, and to ensure that the guality Assurance Hotline promptly and completely addresses all concerns directed to the Hotline.
As we understand the highlights of the areas you indicated you will focus on, you intend to ensure that the root causes of problems are fully understood and developed, in part by demanding a strong involvement by your engineering personnel; you intend to require System Engineers to be responsible for their systems, and will provide the System Engineers with the support necessary for them to be effective; you intend to require your guality Organization to be increasingly more demanding and critical; you intend to ensure that management and the gA Hotline are responsive to employee concerns by continually reenforcing that concept from top levels of management; and you intend to take actions to significantly strengthen your training and maintenance programs.
We support your actions in these areas and look forward to reviewing your progress in a few months time.
