'f7/[ O Projects Section 1 Inspection Summary Inspection on November 5-30, 1979 (Report No. 50-483/79-16)

Areas Inspected: Routine inspection by tie IE Regional Resident Inspec-tor (RI) of safety related construction acLivities including concrete placement, protection of the reactor vessel and other components of the nuclear steam system supplier; (NSSS), material lay down and storage areas; welding of reactor coolant piping, drt; wing cortrol, and general area housekeeping. This inspection involved 82 inspector hours by one NRC inspector.

Results: Of the areas inspected, one item of noncompliance was identified (deficiency) failure ta follow prescribed procedure.

Reportable Deficiency 50.55(e) Item NSSS Lateral Support Anchor }.It Nuts On November 30, 1979, the licensee reported a possible problem with out of tolerance threads for lateral support anchor bolt nuts. This item will be considered unresolved (483/7-16-01) until resolved by Union Electric and accepted by the NRC.

Functional or Pre gram Areas Inspected 1.

Plant Tours One or more plant areas were toured several times each week during the reporting period to observe general construction practices; area cleanliness and storage condition of equipment and piping.

Two tours were made on the second work shift.

Items of noncompliance or deviations are discussed in paragraph 2.

2.

Reactor Coolant Pressure Boundary Pipe Welding Activities

.

The inspector observed the following welding activities:

a.

Reactor Coolant System Weld No. 2BB-01-F001 on drawing M-03BB1.

It was determined that, (1) work was being conducted in accord-ance with the traveler; (2) proper welding materials were used; (3) welding procedure requirements were being met; and (4) the work area was free of weld rod-stubs.

b.

Reactor Coolant System Weld No. 2BB-01-F002 on drawing M-03BB1.

It was determined that, (1) work was being conducted in accord-ance with the traveler; (2) proper welding materials were used; (3) the joint fit up was correctly made; (4) the system was purged and the oxygen content of the purge gas neasured at less than 11% using oxygen indicator WQ-068; (5) the oxygen indicator was in calibration.

During the subsequent observation of welding the root pass, it was determined that a gas flow meter was not installed to moni-tor the system purge during welding. Daniel International weld-ing procedure specificction requires that, "during welding, the purge flow rate be reduced to a minimum of approximately 1 CFil and continue until the weld is 3/16" thick."

This is in noncompliance with the requirements of 10 CFR 50, Appendix B, Criterion V.

The licensee acknowledged the requirement and took immediate ac-tion to correct the finding. Subsequently, flow meters were observed in use for system purges during welding.

Licensee action is considered complete.

Items of noncompliance or deviations were identified as discussed above.

Concrete Placement

During the reporting period, three concrete placements were observed.

Placement 2C164501 (Auxiliary Roof slab).

For each of the placements listed above, some or all of the fol-lowing conditions were noted.

(1) Block out locations were as specified in the drawings.

,

(2) The forms were secure, leak tight, and had been wetted prior to the placement.

(3) The preplacement inspection had been coupleted and was u

in order.

(4) Quality Control inspectors were assigned to each crew in addition to those assigned to perform periodic concrete testing.

(5) Concrete testing was performed at the required frequencies.

(6) Adequate numbers of crews and personnel were assigned to make the placement.

(7) The placement was satisfactory.

It was noticed however, that during placement of the Auxiliary Building roof slab, a craftsman was using a vibrator to move concrete. This was brought to the attention of the QC inspector and the individual. The condition was immediately corrected. No misuse of vibrating eqaipment was noticed during the com-pletion of the pour or on subsequent pours.

(8) Surrounding equipment was protected from any concrete spill during placement or the subsequent use of curing water.

(9) Curing was monitored and found to be satisfactory.

Embed Reinforcement The inspector observed the welding of anchor rods on two embed plates The nonconformance report instructions were checked against AWS DI.1 Requirements. Welding and inspection requirements were correctly specified.

Drawing Control During the inspection period, drawings were checked in the containment and 3.uxiliary Buildings to determine if the latest revisions were in use by the craftsman.

Providing Public Information The inspector met with a reporter representing the Columbia Tribune to discuss Nuclear Regulatory Commission activities at the Callaway Construction site. Articles subsequently appeared in the Columbia Tribune of Columbia, Missouri and the Kingdom Daily News of Foulton, Missouri.

Unresolved Matters Unresolved items are matters about which more information is required in order to ascetain whether they are acceptable items, items of noncompli-ance or deviations. Unresulved items disclosed during the inspection are discussed in the paragraph describing the reportable deficiency (50.55(e)).

Exit Meetings The resident inspector attended the exit meetings conducted by RIII in-spector C. Erb on December 13, 1979 and H. Livermore and Z. Cordero on December 20, 1979. (NRC Inspection Reports No. 50-483/79-14 and 15).

The resident inspector met with licensee representatives denoted under persons contacted on December 6, 13, and 20, 1979. The inspector sum-marized the scope and findings of the inspections performed. During the meeting of December 6, the licensee was informed of the information the NRC expects to receive when a bomb threat is receised at the Callaway site.

The licensee acknowledged the findings reported in previous paragraphs.
