Appendix A NOTICE OF VIOLATION Union Electric Ccapany Docket No. 50-483 Based on the results of the NRC inspection conducted between December 3 and December 21, 1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as noted below.
The item is a deficiency. See the details of this report for a full description of the deficiency.
10 CFR 50, Appendix B, Criterion V, and SNUPPS PSAR, Chapter 17 para-graph 17.1.5 requires in part that, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, or a type appropriate to the circumstances and shall be accomplished in accord-ance with these instructions, procedures, or drawings."
Daniel International welding procedure N-8-8 paragraph 4.8 requires in part that (a) prior to welding the piping assembly be purged at a flow rate of approximately 25 CFH until six volume changes have occurred or the oxygen content has been analysed as below 1.0% and (b) during welding the purge flow rate shall be reduced to a minimum of 10 CFH and continue.
Contrary to the above, procedures were not followed in that the flow rate of purge was not measured during the welding of the root pass of weld
- 2-BB-01-F002 in the pipe between the presserizer and the reactor coolant piping.
