Licensee Action on Previously Identified Items (0 pen) Infraction (50-461/S0-02-04) The licensee is not taking prompt corrective action to preclude repetition of items such as Nonconformance Reports and found that in many instances that no corrective action was apparent. These were brought to the attention of the licensee, and a representative of Baldwin Associate's Technical Services and it was found that they were in nonconformance with their procedures.
This item remains open pending action by the licensee to assure that corrective action is taken to preclude repetition.

(Closed) Unresolved Item (50-461/80-06-06) General Electric audit of Reactor Controls was considered to be lacking in objectivity.
The licensee stated that this matter would be reviewed during the j
next audit of G.E.-
The inspector reviewed IPC's Audit No. 80-04 l
dated August 4 through 7, 1980 which stated that an audit of RCI l
was performed in July 1980 at the Limerick Site in accordance with l
G.E.'s procedures. However, IPC's audit did not address the objec-tivity of this audit.
l The inspector considers this item closed as RCI is no longer at the Clinton site.

V Revision 16, dated October 27, 1980, " Procedure Specification for the Procurement, Storage, and Control of Welding Filler Materials,"
Paragraph 6.2.1 which was changed to reflect the different require-ments of ASME and AWS welding.
The inspector considers this item closed and in conformance with IPC's response letter (U-0200, Q 37-80(11-12)-6) to RIII.
(Withdrawn) 10 CFR 50.55(e) report (81-02) Welding in containment (container liner, reactor pedestal, and bio-shield structures) using wrong or unqualified welding procedure specifications. The inspector reviewed CPS investigation, " Potential Deficiency 10 CFR 50.55(e)
Investigation Control of Special Processes #81-02," L14-81 (04-20)-6.
Y11412, EEC-216-IPC, which stated the deficiency and found that al-though wrong welding procedures were used the essential variables were the same (preheat, interpass, electrical characteristics). The actions to preclude this problem from reoccurring (not accumented on the appropriate NCR's) was to, (1) Discipline engineers have been directed to specify the type of material to be welded (i.e., A-307 to SA-588) on all new travelers or amendments to old travelers.
P-1 to P-1, etc. will no longer be acceptable. Reference: JEF-126-81, CAR #065, Corrective Action.
(2) Technical Services will continue to specify welding procedures; however, a matrix has been developed, which describes the acceptable welding procedures for the various combinations of base materials, to assist in assuring that the appropriate welding procedure has been specified. Reference: CAR

(3) Construction and engineering will also utilize the welding procedure matrix to assist in assuring that proper weld procedures are being used and specifying the types of material to be welded respectively. Reference: CAR #065, Corrective Action, JEF-125-81.
(4) Welding of base material combinations not previously covered under procedures approved to specifications have been submitted and approved. Reference: Revision. 10, BTS 499, N-ASTM-A Special, PQR's 90, 91, 92., 93, 94.
The inspector agrees that this item be withdrawn.

IE Bulletins (Closed) 80-20
" Failure of Westinghouse Type W-2 Spring Return to Neutral Control Switches." The inspector reviewed IPC's letter to RIII (11-0183, LO-80 (0616)-0) which stated that when breaker is in the connected position the control switches are bypassed. Neutral contacts like those attended to in subject bulletin are not used.
Items 2 and 3 are not applicable to Clinton. This was verified and documented in letter from S&L (SLEI-3215, File No. 21FE, dated September 4,1980)
to IPC.

" Valve Yokes Supplied by Malcolm Foundry Company, Inc."
The inspector reviewed IPC's letter Y-6354, LO 8-81 (04-07)-6, which documents the review of all valve contracts to verify that no Malcolm Foundry castings, and IPC's letter (U-0224, LO 8-81 (04.-06)-6), to RIII stating that no active safety related valves with parts cast by Malcolm Foundry.

" Problems with HPCI Turbine Oil System." The inspector reviewed IPC's IE Circular status sheet which stated that Item 1 of subject circular is applicable and tests of turbine lube oil will be done on a periodic basis.
Item 2 is not applicable as CPS does not have hydraulic stop valves.

" Emergency Diesel Generator Lube Oil Cooler Failures." The inspector reviewed IPC's IE Circular status sheet which stated that corrosion inhibitors to be used at CPS will be in accordance with EMD recommendations from evaluation of the tube sheet failures at a NO-1.

Sampling and instructions are used to preclude problems and incorporated into indoctrination and training program.

" Design Problems Involving Indicating Pushbutton Switches Manufactured by Honeywell Incorporated."
The inspector reviewed a GE letter dated March 23, 1981, IP-1678 stating that CPS does not incorporate Honeywell (Series 2) switches in any NSSS or General Electric, San Jose supplied B0P hardware and S&L letter dated March 27, 1981, File No. 21IE, stating that BOP systems do not use the subject indicating pushbutton switches in any safety related functions.

Functional or Program Areas Inspected Review of Welder Qualifications-Visual Examination of Weldments The inspector visually examined weldments IDG2-3, IRHR 17-9A, IRH 17-10, RH-07, 1DP 4-1 and various weldments on the 0-degree structure.
In addition, eight (8) welder identifications on the subject weldments
'ere traced to their qualifications.
No items of noncompliance or deviations were identified.
6.
Exit Interview f
The inspector met with licensee personnel (denoted in Paragraph 1) at the conclusion of the inspection on May 15, 1981. The inspectors sum-marized the scope and findings of the inspection, which were acknowledged by the licensee. The inspector also discussed infraction 50-461/80-02-04 in that, procedures had been changed as a result of the infraction, but are not being followed. A representative of Baldwin Associated Technical Services made a commitment, for the licensee, to instruct / train the individual to follow the procedure and document all corrective actions
