The licensee stated that further action is being taken to inspect other small bore pipe fittings from the same manufacturer that are located within the primary pressure boundary to assure that a similar problem does not exist.

IE Bulletings (Closed) Bulletin 79-09: Failures of GE Type AK-2 Circuit Breaker in Safety Related Systems.
' Illinois Power Company-(IPC) response (File No. U-0133, dated March 24, 1980)
states that no GE Type AK-2 circuit breakers will be used with any safety related services at Clinton 1 and 2.
The inspector observed appropriate correspondence from General Electric (GE) and Sargent and Lundy (S&L) verify-
ing the licensee response.

IPC response (File No. U-0100, dated July 26, 1979) states that Westinghouse DB-75 and DB-50 circuit breakers will not be used with any safety related services at Clinton 1 and 2.

Operating License or Construction Permit requires a review of test procedares and test schedules for all safety-related circuit breakers which utilize dashpots for overcurrent time delay protection. The IPC response did not sufficiently address this item.
(0 pen) Bulletin 79-23:
Potential Failure of Emergency Diesel Generator Field Exciter Transformer.
IPC response (File No. U-0110, dated October 31, 1979) states that the Exitation Power Transformers used with Clinton diesel generators will be operated with floating primary neutrals thus item 1 of action to be taken by licensee is not applicable.
In response to item 2 IPC states that the preoperational test program to demonstrate full-load carrying capability of the diesel generators in accordance with Reg. Guide 1.108, Revision 1 is scheduled for December 1981. The inspector will review the subject preoperational procedure which is scheduled to be written in March, 1981.
(Closed) Bulletin 79-25: Failure of Westinghouse BFD Relays in Safety Related Systems.
IPC response (File No. U-Oll5, dated January 3,1980) states that the subject relays will not be used with any safety related services at Clinton Power Station. The inspector observed appropriate correspondence from GE and S&L verifying the licensee response.

IPC response (File No. U-0122, dated February 7,1980) indicates that addi-tional vendor investigation to ensure that the subject switches have not been shipped nor are to be shipped is yet to be completed.
(Closed) Bulletin 80-09: Hydroamotor Actuator Deficiencies IPC's Bulletin Status Sheet, states that cps does not have ITT General Controls Models AH-90 and NH-90 series Hydramotor actuators in safety related service.

Status Sheets states that, access to perform inspections on core spray spargers is in the design stage. Spargers will be visually examined as part of the ISI program.
ISI vendor is aware of the potential IGSCC problems and will assist IP in inspection procedures as necessary.

(Open) Bulletin 80-16: Potential Misapplication of Rosemont, Inc. Models 1151 and 1152 Pressure Transmitters with Either "A" or "D" Output Codes.
IPC response (File NO. U-0173, dated August 23, 1980) states that the subject transmitters may have been installed by GE on NSSS safety systems and that GE has instructed field personnel to identify any of the subject transmitters which may be onsite. The inspector was informed by the licensee that GE is still in the process of identifying the subject transmitters.
(Closed) Bulletin 80-19: Failures of Mercury-Wetted Matrix Relays in Reactor Protective Systems of Operating Nuclear Power Plants Designed by Combustion Engineering.
IPC response (File No. U-0177, dated August 26, 1980) states that mercury-wetted switches are not used in Clinton nuclear safety related systems in Units 1 or 2.
The inspector observed appropriate correspondence from GE and S&L verifying the licensee's response.

(0 pen) Bulletin 80-20: Failures of Westinghouse Type W-2 Spring Return to Neutral Control Switches.
IPC letter (File No. U-0183, dated September 16, 1980) stated that while the subject switches are supplied on all Class 1E 4160V and 6900V circuit breakerr, the application is only for indication of breaker in test status; the neutral contact is not used; no safety related application of the switch exists.
Further inspection is required to verify the licensee's response.
(Closed) Bulletin 80-23: Failures of Solenoid Valves Manufactured by Valcor Engineering Corporation.
IPC response (File No. U-0204, dated December 12, 1980) states that no subject solenoid valves will be installed at Clinton. The inspector ob-served appropriate correspondence from GE and S&L verifying the licensee's response.

IE Circulars (Open) Circular 80-16: Operational Deficiencies in Rosemont Model 510DU Trip Units and Model 1152 Pressure Transmitters.
The licensee has not determined if Clinton has installed or plans to install the Rosemont Model 510DU Trip units nor has the licensee completed this deter-mination for the Model 1152 Pressure Transmitters.
(Closed) Circular 80-23: Potential defects in Beloit Power Systems Emergency Generators.
Licensee has determined that Clinton Power Station does not have Beloit Emergency Generators at the Site.
Licensee's memo (7fle No. Y-5872, dated November 19, 1980) states that the Clinton Emergency Power units are composed of GM-EMD diesels coupled with Ideal Generators.

Observation of Electrical Work Activities a.
The RIII inspector observed the following installed conduit hangers as located per S&L drawing No. E30-1004-01A-E1H, Revision D:
Hangers JS-F-2 JS-F-3 TS-1A TS-1B JB-2 JS-5 Where applicable, the above supports were installed in accordance with Baldwing Associates Traveler No. E30-1004-0A-JS-F-2, Revision 1, No.
E30-1004-01A-JS-F-3, Revision 1; No. E30-1004-01A-TS-1A, Revision 00; No. E30-1004-01A-TS-1B, Revision 00; No. E30-1004-01A-JB-2, Revision 00, No. E30-1004-01A-JS-5, Revision 00.
The components, dimensions, tolerance, number of assemblies and spacing were as specified except as follows:
(1) The inspector observed the traveler for hanger JB-2 and determined that there was no plan view to provide orientation of pall box dimensions. The inspector observed nine (9)
other travelers each of which included hanger plan views.
The licensee stated that a plan view is to be included for each hanger traveler.

k (2) The inspector observed that a clip dimension on hanger JS-5 was 35" x 25" x 5" x 8" instead of the required 3\\" x 2 " x
\\" x 6".
The licensee stated that this hanger would be reinspected.
Hanger velds exhibited no lack of fusion, cracking, porosity, overlap, or excessive undercut. There was no sign of rust on any of the welded joints. The inspector was unable to inspect all welds due to installation of fire proofing material at the hanger locations.
No items of noncompliance were identified.
b.
The RIII inspector observed sheet 3 of 4 of hanger Field. Change Request (FCR) 6196 installed on a rack of working drawings located in the Control Building. During the inspection of hanger E30-1004-01A-JS-F-2, sheet 3 of FCR 6196 was mistaken for S&L hanger location drawing E30-1004-01A-E1H, Revision D by licensee personnel. The inspector observed that the two drawings are identifical except for the drawing revision number and a handwritten notation to identify sheet 3 of FCR 6196. The inspector observed several other sheets

of various hanger FCRs on the same rack of working drawings. The licensee stated that appropriate corrective action would be imple-mented. This matter is unresolved.

c.
The RIII inspector questioned the licensee concerning the implementa-tion of S&L design changes to completed electrical raceway hangers.
Specifically, the inspector inquired as to procedures the licensee would follow to identify, prior to completion of turnover, all out-standing design changes to previous 1/ installed hangers. The licensee stated that this concern was undergoing review and that appropriate action would be implemented. This matter is unresolved.

The RIII inspector observed installed electrical penetration IEE28E.
The Containment Building and Auxiliary Building ends were covered with a fireproof material which also provided a shield against dust and debris. -No damage was observed; the length of the flange bolts was as specified; the penetration was located per the location drawing; the pressure gage indicated proper pressurization.

The RIII inspector observed the in process lasta11ation of electrical penetration IEE18E. The penetration was installed with care and precision in accordance with Conax Proc'edure No.
IPS-272, Revision A.
A Baldwin Associates inspector was present during the installation activities.
Areas inspected included the condition of the penetration after transport and uncrating, prelininary dimensional check and subsequent dimension adjustment, location, and orientation, allignment/ configuration, and cleanliness /
fitup.

2.
Record Review of Electrical Work Activities a.
The RIII inspector reviewed Baldwin Associates Traveler JV-667 and Quality Control Support Fabrication / Installation Checklist JV-668 pertaining to the aforementioned conduit hangers. The records identified the inspector, welder, type of observation, results, acceptability, and identified related documents. The records were identifiable, retrievable, and appropriate for the activities inspected.
b.
The RIII inspector reviewed the qualification records of the visual weld inspector responsible for inspection of the previously mentioned hangers. The licensee indicated that the inspector cas qualified per ANSI 45.2.6 at the Level II position. The inspectors qualifica-tion records stated that the four years of previous inspection and test experience required by ANSI 45.2.6 were waived substituting 120 hours0.00139 days <br />0.0333 hours <br />1.984127e-4 weeks <br />4.566e-5 months <br /> of on the job training and 80 hours9.259259e-4 days <br />0.0222 hours <br />1.322751e-4 weeks <br />3.044e-5 months <br /> of self-study courses.
The inspector's qualifications were limited to fillet welds. The records included appropriated practical and visual examinations which the inspector passed.
c.
The RIII inspector reviewed the qualification records of the welders of the aforementioned conduit hangers. The records indicated proper qualification per AWS Dl.1.

d.
The RIII inspector reviewed the installation record for penetration IEE28E. The installation was properly documented on travelor 1EE28E, Revision 0 which was undergoing final review by Technical Services and Quality Control. The inspector observed NCR 3474 dated July 21, 1980, written during initial installation of electrical penetrations. The NCR stated that all of the 7/8" bolts received from Conax to be used to torque down the primary header plate to be attained. Disposition was to send the subject bolts back to Conax for rethreading. The licensee informed the inspector that the rethreading process was complete for all installed penetrations.
No items of noncompliance were identified.
