Inspection Summary

,

Inspection on June 25-27, 1980.(Report No. 50-440/80-10)

Areas Inspected: Reactor Coolant Pressure Boundary and Safety Related Piping work activities, welding, procedures and records; Safety Related Piping Support welds. The inspection involved a. total of 20 onsite hours by one NRC inspector.

Results: Of the seven areas inspected, no apparent items of noncompliance or deviations were identified in five areas; two items of noncompliance were identified in two areas (infraction - failure to meet welding procedure re-quirements, paragraph 3; infraction - inspection failed to identify noncon-forming item, paragraph 10).

Reactor Coolant Pressure Boundary Piping - Welding Procedures, Speci-fications and Quality Assurance Procedures a.

Quality Assurance Procedures The inspector estsblished that General Electric Company Installa-tion and Service Engineering Division (I&SE) procedures relative to the welding of the Reactor Coolant Pressure Boundary Piping have not been revised since the last review (IE Inspection Report No. 50-440/80-08).

Welding Procedures

.The inspector reviewed I&SE welding procedure No. 8.9.4.1 Revision 4 and its qual.ification records Nos. 306 and No. 310.

It was determined that ASME Section IX requirements were met.

Reactor Coolant Pressure Boundary Piping - Welding Material Control a.

Review of Documents The inspector reviewed the following documents:

(1) I&SE Procedure No. GWP-1010, Revision 2, " Weld Material Control and Storage."

I&SE Procedure No. GEP-AP-onr6, Revision 1, " Site Procure-ment Document Control."

(3)

I&SE Procedure No. GEP-G21-0005, Revision 1, " Materia?

Control."

(4) Receiving Inspection Reporc, Purchase Order and Material Certification for ER308L welding rods with heat No. 05845 and lot No. 31915; E308L-16 electrodes with lot No. 8M13C and No. OD21B-2.

It was determined that the above documents met the PSAR, 10CFR 50, Appendix B, and the applicable code requirements.

Control of Materials The inspector toured I&SE welding material issuing station located l

l at the Intermediate Building.

It was determined that (1) the weld-ing materials are properly identified and segregated; (2) the tem-

perature of the rod ovens is maintained; (3) records are properly kept and (4) issuance and return of welding materials are controlled in accordance with approved procedures.

No items of noncompliance or deviations were identified.

3.

Reactor Coolant Pressure Boundary Piping - Observation of Welding Activities The inspector observed welding of the Reactor Recirculation System weld No. B33-1-A6 on traveller No. T1-B33-04 and weld No. B33-1-B6 on traveller No. T1-B33-06.

It was determined that (1) work was conducted in accordance with traveller; (2) proper welding materials were used; (3) welding procedure requirements were met except a pass of the weld bead width on weld No. B33-1-B6 exceeded the procedure requirement; (4) work area was free of weld rod-stubs; and (5) physical appearance was acceptable.

Item 3 represents an item of noncompliance as identi-fied in Appendix A (440/80-10-01).

Except as noted, no items of noncompliances or deviations were identified.

Reactor Coolant Pressure Boundary Piping - Weld Heat Treatment a.

Review of Preheating Procedure The inspector reviewed I&SE Procedure No. GWP-1000, Revision 15, and determined that preheating and monitoring methods are speci-fled and acceptable.

b.

Observation of Preheating While observing welding activities for items stated in paragraph 3 of this report, the inspector determined that preheat met the pro-cedure requirements.

No items of noncompliance or deviations were identified.

5.

Reactor Coolant Pressure Boundary Piping - Welder Qualif_i. cation The inspector reviewed the following documents:

a.

I&SE Procedure No. GWP-1003, Revision 9, " Welding Performance Qualification."

b.

Qualification records of welders performing welding on welds stated in paragraph 3 of this report.

The inspector determined that the above documents met the

requirements of ASME Section IX.

No items of noncompliance or deviations were identified.

Reactor Coolant Pressure Boundary Piping - Observation of Work and Work Activities The inspector observed the rigging and protection of the Reactor Re-circulation System pipe spool No. 762E843-G008A.

It was determined that work activities were performed in accordance with the applicable procedures and good construction practices were adhered to.

No items of noncompliance or deviations were identified.

7.

Safety Related Piping - Observation of Work and Work Activities The inspector observed the fit-up and alignment of Residual Heat Re-moval System welds No. 03 and No. 04 on drawing No.1E12-22.

It was determined that work activities were performed in accordance with the applicable procedures and good construction practices were adhered to.

No items of noncompliance or deviations were identified.

Safety Related Piping - Observation ofWelding Activities The inspector observed welding of Emergency Service Water System weld No. 03 on isometric drawing No. 1-P45-3 and High Pressure Core Spray System weld No. 02 on isometric drawing No. 1-E22-6.

It was determined that (1) work was conducted in accordance with traveller; (2) proper welding materials were used; (3) welding procedure requirements were

met; (4) work area was free of weld rod-stubs; and (5) physical appear-ance was acceptable.

No items of noncompliance or deviations were identified.

9.

Safety Related Piping - Special Welding Application The inspector observed the grinding of defect, liquid penetrant exami-nation of ground area and weld repair of Residual Heat Removal Sy-tem weld No. 03 on isometric drawing No. 1E12-11.

It was determined that (1) work was conducted in accordance with traveller; (2) procedure requirements were met; and (3) proper materials were used.

No items of noncompliance or deviations were identified.

10.

Visual Inspection of Completed Pipe Hanger Welds The inspector visually inspected welds No. AFW-4 and No. AFW-5 of the Emergency Closed Cooling System Pipe Support No. IP42-123.

It was determined that the undercuts appear to exceed 1/32" and the weld on the back side of the plate is not fully penetrated. This is an item of noncompliance as identified in Appendix A (440/80-10-02).

Exit Interview The inspector met with licensee representatives (denoted under Persons Ceatacted) at the conclusion of the inspection on June 25-27, 1980. The inspector summarized the purpose and findings of the inspection. The licensee acknowledge the findings reported herein.
