Inspection Procedures 30703, 73051, 73753 and 73755 were used as guidance for the inspection.
Results:
General Conclusions and S ecific Findin s:
Nondestructive examinations (NDE) observed were in accordance with the licensee's ISI Program requirements.
In addition, ultrasonic (UT)
examinations of dissimilar metal welds for detection of intergranular stress corrosion cracking ( IGSCC), the subject of NRC Information Notice No. 90-30, were performed by the licensee on the Feedwater (FW) safe-end-to-nozzle welds utilizing a refracted longitudinal wave transducer.

Inservice Ins ection a ~
In serv ice Ins ection Plan 73051 The inspector reviewed the licensee's inservice inspection (ISI)
plan governing the ISI activities during the fifth refueling outage (RF90A).
The ISI plan contained a listing of all items that were required to be examined during RF90A, the method of examination, calibration blocks used, applicable procedures, and certification requirements for personnel, equipment and materials.
The ISI program was found to be in accordance with the requirements of the ASME Code,Section XI, 1980 Edition, through the Winter 1980 Addenda.

The ISI procedures associated with the examinations observed by the inspector during this inspection were:

These procedures were in accordance with the licensee's ISI plan.
Observation of Work and Work Activities 73753 The inspector observed the following ISI examinations being performed on the Reactor Feedwater System:

~IS I M~hd Ultrasonic examination (UT) attributes that were evaluated and found to be consistent with the approved procedures and ASME Section XI requirements were:
the type of apparatus used, scanning technique, extent of coverage, UT beam angle and frequency of the search unit, calibration requirements, evaluation limits and recording of indications.
The magnetic particle examination activities observed included a demonstration of the yoke lifting power, magnetic powder application, and magnetic particle inspection of the weld area.
All work observed was in accordance with ASME Section XI requirements; no deficiencies were noted.
Also the qualification and certification records for the ISI examiners and the equipment and material certifications were reviewed by the inspector.

Data Review and Evaluation (73755 The inspector reviewed the magnetic particle and ultrasonic examination data sheets for the nondestructive examinations observed during this inspection.
No reportable indications were reported on the data sheets which were reviewed by the ANII and the licensee's ISI Level III examiner.
The inspector also reviewed the ultrasonic examination data sheets for the Feedwater (FW) safe-end-to-nozzle welds examinations that were performed using a refracted longitudinal (RL) wave search unit.
The FW safe-end-to-nozzle welds are dissimilar metal welds ( Inconel and carbon steel) of the type addressed in NRC Information Notice No. 90-30 entitled "Ultrasonic Inspection Techniques for Dissimilar Metal Welds", dated May 1, 1990.
Essentially this information notice says that the conventional shear wave mode ultrasonic transducers are not reliable for detecting intergranular stress corrosion cracking (IGSCC) in dissimilar metal welds containing Inconel 600 materials.
The recommendation is for use of RL waves for IGSCC detection and sizing.
General Electric notified BWR owners about this problem in 1989 and subsequently was contracted by the licensee to perform the dissimilar weld examinations.
The data sheets reviewed by the inspector were for the following FW welds examined during this outage:
BE-9, BE-10, BE-11, and during the 1989 outage BD-11.
No evidence of IGSCC was reported.
The qualifications of the GE personnel performing these examinations utilizing the RL wave search units were reviewed.
The Level II UT examiner, in addition to being qualified to the American Society for Nondestructive Testing Recommended Practice No. SNT-TC-1A, also was Electric Power Research Institute (EPRI) qualified for:
1)
UT operator training for the detection of IGSCC, 2)
UT operator training for weld overlay examination using a
refracted longitudinal wave transducer, and 3)
UT operator training for sizing IGSCC.
No violations or deviations were identified.

The inspector met with licensee representatives denoted in paragraph 1 on May 18, 1990.
The scope and findings of the inspection were discussed as described in this report.
