The Control Room, Technical Support Center, Operations ' Support Center, standby activation of the Emergency Operations Facility, onsite sampling and survey teams and offsite monitoring teams were observed.

Open Items and License Condition Items were reviewed for current status.

The inspection involved 67.5 inspector-hours onsite by five NRC inspectors.

Results: No items of noncompliance or deviations were identified.

Licensee Action on Previous Inspection Findings (Closed) SER item (373/81-0012B; 374/81-00-02): 10 meter level Measurements System. The licensee has completed a six month study to evaluate the representativeness of the siting of this instrument to verify that nearby buildings do not inhibit 10 meter surface winds measurements. The study indicated no significant distortions due to building effects. This finding was requested as per SSER, No. 2, Page 22-5.

(Closed) Open item (373/81-14-26; 374/81-09-26): Calibration factors for LZP-1200-2 and LZP-1200-4.

The licensee has developed noble gas and liquid effluent calibration factors and incorporated them into the LZPs. The inspectors verified that these factors have been developed, ! and reviewed these LZPs. The LZPs, which are currently in an onsite j safety review, will be implemented in the near future.

(Closed) SER item (':73/81-00126 Items 1 and 2): Improving Licensee l Emergency Preparedness. The licensee conducted an onsite (small ( scale) exercise with full scale participation of offsite authorities including the State of Illinois, Grundy County and LaSalle County on

April 14 and 15, 1982.

The exercise. included a test of the prompt public notification system.

. ' A test of the prompt public notification system, which was observed by the Illinois State Emergency Services and Disaster Agency (ESDA) representative and an NRC inspector was also conducted on January 26, 1982. Two sirens did not activate owing, appareatly to bent antennas.

These were fixed and retested during the April 15, 1982 test.

(Closed) Open item (373/81-14-27(B.1)); SER items (373/81-00126 (B.1)) and (373/81-00127) Improving licensee Emergency Preparedness. The licensee has submitted their Offsite Dose Calculation System to NRR, which included a description of the dose calculational methodology for Class A and B models.

It is currently being reviewed by NRR.

(Closed) Open item (373/81-14-23; 374/81-09-23): Installation of First Aid equipment.

First aid kits, stretchers and blankets have been installed around the site as per LZPs.

(Closed) Open item (373/81-14-24; 374/81-09-24): Installation of monitors. Area and process radiation monitors, used for EALs have been installed and initial calibrations performed. However, some of the monitors are in need of additional calibration. For Emergency Planning purposes, the EP Section considers this item closed.

(Closed) IE Bulletin 80-15 (373/80-30-02): The licensee has provided vital power backup for the ENS phone system, has satisfactorily tested it and esolved the frequency disparity which was initially identified as indicated in IE Report 50-373/82-01; 50-374/82-01.

(0 pen) Open item (373/81-14-31; 374/81-09-31): Installation and calibration of PING-3 in EOF. The inspectors verified that the PING-3 had been installed in the EOF; however, the unit has not been calibrated.

(Open) SER item (373/81-00126 Items 4.A, 4.B.2, and 4.B.3)) and Open item (373/81-14-27A, B.2 and B.3; 374/81-09-27A, B.2 and B.3): Im-proving Licensee Emergency Preparedness. The licensee has completed installation of the Class A, B and C models for the LaSalle station except for the prime computer printer, which has to be installed in the control room.

Further, a procedure for the acquisition of real time and past time (previous 12 hours) had not been prepared at the time of the inspection. This procedure is currently being prepared by the licensee. After the prime printer installation and the pro-cedure completion the licensee's dose assessment methodology will meet the guidance of Appendix 2 of NUREG-0654.

(0 pen) Open item (373/81-14-25; 374/81-09-25): Installation and testing of microwave voice communications. The licensee has installed and tested the backup microwave voice channel (grey phone) in the l control room, TSC and CCC; however, this phone has not been installed in the EOF as required.

Alarms will automatically print out on the alarm printer. This item remains open until these monitors have been calibrated and station training relevant to their use completed and verified by the NRC.

(0 pen) Open item (373/80-53-03):, Site assembly drill. The licensee y has not performed a site assembly drill to test personnel account-ability.

Neither was it demonstrated during the April 14 and 15, 1982, exercise. This drill must be conducted prior to initial criticality.

(Closed) Open item (373/81-14-17; 374/81-09-17) (Reopen) SER item (373/81-00129): Rad / Chem Technician (RCT) Training.

The licensee has not completed RCT training prior to back shift cover-age to demonstrate proficiency in the o'ojectives stated in License

Condition 2.C(24) (a through g).

General A small scale exercise of the Licensee's Emergency Plan was conducted

on April 14 and 15, 1982, at the LaSalle County Station. The exercise was scheduled to keep the level of the incident at an Alert Status for the first day with participation prescribed by the GSEP, and escalate to a Site Emergency early the second day in order to fully involve the State and counties.

Licensee involvement the second day was to . ' read scripted messages portraying the major events and react to ques-tions from State and local agencies. The exercise was a test of the licensee's, and the State and local agencies' response capability to a scenario which involved a small break LOCA and resultant release of radioactive gases. Attachment 1 describes the scenario for April 14, 1982, when station personnel were actively participating. The NRC did not observe licensee actions on the second day, April 15, 1982.

General Observations a.

This exercise was conducted in accordance with 10 CFR 50 Appendix E requirements, as modified in SSER No. 2 (NUREG-0159), page 22-4 using the Commonwealth Edison GSEP and appropriate Emergency Plan Implementing Procedures. The exercise was small scale and did not escalate beyond and the \\lert level with regard to the response of the LaSalle Station Ier anel.

b.

Licensee and five NRC ineyectors observed and critiqued the exercise.

The licensee held a critique on (,pril 14, 1982, immediately after the exercise. The NRC and the licensee identified items which . should be considered for improvement. The items are discussed in " Paragraph 5.

Summary of Areas for Improvement Problems identified by NRC observers and discussed during the exit interview include areas in which additional attention should be given.

They are listed below: a.

Public address announcements should be made regarding changing conditions of plant status i.e., Alert, etc.

b.

The Station Director should have a multiline teleph6ne at his station to eliminate the need for leaving his station to talk to other non-TSC areas such as the control room or EOF.

c.

The OSC director should have an assistant to ease his workload.

d.

Public address announcements to activate the TSC should also include activation of the OSC to assure prompt reporting of assigned OSC personnel to their station.

e.

A method of assuring that station personnel for each shift are aware that they are to report to the OSC when it is activated should be procedurized.

f.

A status board should be provided for the OSC.

g.

Personnel monitoring and contamination control should be provided at the HRSS room.

h.

The dedicated communicator in the control room should be provided with a telephone headset for communications with the TSC.

Assure initial and continuing habitability monitoring of the OSC.

J.

Improve communications from the High Range Sampling System room to the TSC and OSC.

Specific Observations a.

Control Room The resident inspector observed the conduct of the control room personnel during the exercise. The Operators, Shift Foreman and Shif t Engineer appeared to be knowledgable in the use of their emergency procedures and appeared to understand their responsi-bilities. The turnover of responsibility frcm the Station Control Room Engineer to the Shift Foreman was very comprehensive.

The dedicated control room communicator was very busy at the onset of the exercise. At times he left the control room center desk to give information to the Shift Engineer, as no headset was available for him to maintain communications and have mobility to deliver messages.

. ' Although the staff appeared to be knowlgedeable in performing their functions, they did not know how to access meteorological data from the computer.

b.

Techical Support Center (TSC) Inspectors observed the licensee's response activities in the Technical Support Center (TSC) during the exercise. Due to the constraint placed on the licensee by the State involvement, activation of the TSC was premature and therefore could not be accurately evaluated. Activities appeared to be coordinated and licensee personnel appeared trained in carrying out their func-tions. The scenario was not prepared in strict adherence with the facility parameters which led to confusion regarding plant status data routinely reported to the TSC staff and trended.

In addition, two " Flip Charts" were used to display plant information beyond normally trended Plant status and radiological information.

An Eberline PING-3 was activated to provide continuous habitability monitoring.

It was noted that although the TSC is functional in its design and has adequate space and facilities, the Station Director had to leave his working station to answer incoming calls to the TSC.

A multiline telephone would eliminate this problem.

c.

Emergency Operations Facility (EOFj The interim nearsite EOF was not formally activated but only put on alert due to the scenario makeup. However, two offsite environs monitoring teams, some emergency workers, and the Environs Director operated out of the EOF. They prepared selected equipment and checked hardware dedicated to this facility, includ-ing telephones, equipment lockers, the microwave radio telephone link between the ERFs, and the computer terminal for dose assess-ment.

An Eberline PING-3 provided continuous habitability monitor-ing. These operations were implemented quickly and orderly, d.

Of fsite Monitoring Team Each team consisted of two Rad / Chem Technicians and one Communicator.

Team No. 2 was observed by the NRC inspector. Team members were equipped with personnel monitoring devices (film badge and a 1 R and 10 R self reading dosimeter) and continuously monitored back-grounds while in transit between sampling sites. Necessary equip-mont and implementing procedures were available to the team.

Communications between the team Communicator and the Environs Director were timely and clearly transmitted. The team used li-consee and State EPZ maps and referred to sector and letter, and road designations to identify monitoring locations.

Confusion was noted once between a licensee and State designation for the

_- _ _ __ __ .. . .. - . I " same location. Field monitoring included direct radiation mea- , i surements, TLDs posting and water, vegetation and soil sampling.

Monitoring results, except for background readings, were simu-lated. However, the licensee demonstrated capability and know-ledge of offsite monitoring techniques.

Operations Support Center (OSC) The OSC was activated in a timely manner and the OSC director notified the control room of its availability within five minutes of its activation. Only one Rad / Chem Technician reported initially , l and only after a request by the director did the complete Rad / Chem complement arrive. The OSC director provided routine verbal updates. Throughout the exercise it was apparent that he had too many tasks to perform and should be provided with an assistent.

It was not apparent that habitability monitoring was performed in the OSC.

f.

Post Accident Sampling The inspector accompanied the sampling teams during the collec-tion of three samples. The teams were briefed prior to leaving the OSC and carried survey instruments for personal monitoring.

. Teams collected samples and brought them to the counting room.

Procedures for collecting and analyzing samples were available in the sampling and counting rooms. The sample room contained equipment necessary for collecting high activity samples; the counting room had sufficient equipment for analyzing medium to low level samples.

Communications from the sampling room to the OSC were of poor quality owing to building materials shielding radio signals. The inspector also observed that step-off pads and monitors were not located near the sample room to provide personnel monitoring and contamination control.

Exit Interview The inspectors held an exit interview with licensee representatives denoted in Paragraph 1 on April 15, 1982. The licensee agreed to consider the item listed in Paragraph 5.
