Areas Inspected:

Review of inservice inspection (ISI) procedures, program, material and equipment certifications; nondestructive examination (NDE) per-sonnel certifications; IE Bulletin No. 80-01 activities and observe NDE of the cibow to safe end/ nozzles in the high pressure injection lines. This inspection involved a total of 18 onsite inspector-hours by one NRC inspector.

Results: No items of noncompliance or deviations were identified.

Observation of Work Activities a

The inspector observed the work and had discussions with personnel during review of the following activities. These observations

included calibrations, performance of the examinations, and the documentation. All the following NDE was found to be acceptable.

Ultrasonic examination of the elbow to safe end/ nozzles

The inspector reviewed the construction radiographs dated

.

i January 10, 1975, showing the four button welds which hold the thermal sleeve in place on the above elbow to safe ends. B&W re-radiographed the four above elbow to safe ends and it was determined that the thermal sleeves were still in position, the

,

buttons were still all welded in place and that there were no

'

defects noted.

No items of noncompliance or deviations were identified.

Exit Interview The inspector met with site representatives (denoted in Persons Contacted paragraph) at the conclusion of the inspection. The inspector summarized the scope and findings of the inspection noted in this report.
