Subject: Inspection 50-333/88-25-This refers to your letter dated December-13, 1988, in response to our letter dated November 21, 198 On . February 25, 1989 and March 1, 1989 my staff held telephone conversations with Messrs. D. Ruddy and K. Moody of your staff to discuss the objection to the subject violation. :From these discussions, we understand.that at the time-of the inspection there was in dequate information to establish harsh. environ-ment qualification of the tape-wrap cable splice configuration at the FitzPatrick site. FitzPatrick Nuclear Power Plant Docket No. 50-333 Intspection No. 88-25 Reference: (1) NRC Inspection Report No. 50-333/88-25 Dated November 21, 1988 Gentlemen:

As identified and requested by reference (1), the Authority submits the response to Appendix A, Notice of Violatio Notice of Violation As a result of the inspection conducted on October 11-14, 1988, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy)

(1988), the following violation was identiiied:

10 CFR 50.49 paragraphs (f) and (j), respectively, require that (1)

each item of electric equipment important to safety shall be qualified by testing and/or analysis of identical or similar equipment, and the qualification based on similarity shall include a supporting analysis to show that equipment to be qualified is acceptable; and, (2) a record of the qualification shall be maintained in an auditable form to permit verification that each item of electrical equipment important to safety is qualified and that the equipment meets the specified performance requirements under postulated environmental condition Contrary to the above on October 14, the NRC inspectors identified Okonite cable splices in Limitorque operator No. 12 MOV-15, that were not qualified in that they were installed in a condition that was not similar to the qualified configuration. Specifically, the installed half-lapped layers of the tape splice were 1/8-1/4 inc This is substantially less than the 3/8-1/2 inch half-lapped layer required to be in accordance with the qualified configuratio This is a level IV Violation. Subj ect: JAFNPP/50-333/NRCI 88-25 Page 2 Response to Violation The Authority does not agree with this violation. As presented November 29, 1988, at a meeting in the NRC Region 1 offices, the identified Okonite cable splice should not be an environmental qualification (EQ)

concern. The function of the outer black tape, which was observed to have improper overlap, is to provide abrasion protection. Electrical insulation capability is provided by an inner red tape which was applied satisfactorily to meet EQ concerns. Records of the qualification program which confirm this are available at the FitzPatrick sit The identified cable splice was not exactly in accordance with a plant procedure which specifies how this type of splice is to be performe The root cause of the improper splice was that the contract electrician who performed the splice was not properly trained in the implementation l of the governing procedur This type of splice is normally performed i by Authority permanent electricians who have been properly trained. The training program is being upgraded to ensure contract electricians are properly trained in this type of splice. Secondary causes of this event were the difficulty of making the splice properly in close quarters wearing protective clothing (including two pairs of rubber gloves) and inadequate QC inspectio A review was conducted to identify additional splices of this type that were performed during the refuel outage and to independently inspect a sample of them for proper compliance.
