Cook Nuclear Plant Docket No. 50-315 License No.
DPR-58 EA 96-020 During an NRC inspection conducted on December 20, 1995, through Januar'y 16, 1996, violations of NRC requirements were identified; In accordance with the
"General Statement of Policy and Procedure for NRC Enforcement Actions,"
NUREG-1600, the violations are listed below:
A.
Technical Specification 3.5.2 requires, in part, that two independent ECCS subsystems be operable with each subsystem comprised of one operable centrifugal charging pump in Hodes 1, 2, and 3.
With one ECCS subsystem inoperable, restore the inoperable subsystem to operable status within 72 hours8.333333e-4 days
0.02 hours
1.190476e-4 weeks
2.7396e-5 months
or be in Hot Shutdown within the next 12 hours1.388889e-4 days
0.00333 hours
1.984127e-5 weeks
4.566e-6 months
.
Technical Specification 3.0.3 requires, in part, when a Limiting Condition for Operation is not met, except as provided in the associated Action requirements, within one hour action shall be initiated to place the unit in a Hode in which the Specification does not apply by placing it, as applicable, in at least Hot Standby within the next 6 hours6.944444e-5 days
0.00167 hours
9.920635e-6 weeks
2.283e-6 months
, at least Hot Shutdown within the following 6 hours6.944444e-5 days
0.00167 hours
9.920635e-6 weeks
2.283e-6 months
, and at least Cold Shutdown within the subsequent
hours'ontrary to the. above, from 4:31 a.m.
on Harch 15, 1995, until Il:17 a.m.
on July 30, 1995, while in Hodes 1,
2 or 3, the Unit
West (Train B) centrifugal charging pump was inoperable, a period greater than 84 hours9.722222e-4 days
0.0233 hours
1.388889e-4 weeks
3.1962e-5 months
, and Unit 1 was not placed in Hot Shutdown.

on July 10, 1995, until 2:50 a.m.
on July 12, 1995, while in Hode 1,
and for 17 hours1.967593e-4 days
0.00472 hours
2.810847e-5 weeks
6.4685e-6 months
and
minutes on July 19, 1995, while in Hode 3, both centrifugal charging pumps were inoperable and action was not taken within one hour to place Unit 1 in at least Hot Standby within the next
hours, at least Hot Shutdown within the following 6 hours6.944444e-5 days
0.00167 hours
9.920635e-6 weeks
2.283e-6 months
, and at least Cold Shutdown within the subsequent 24 hours2.777778e-4 days
0.00667 hours
3.968254e-5 weeks
9.132e-6 months
.

B.
CFR 50, Appendix B; Criterion V, "Instructions, Procedures, and Drawings," requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances.
Contrary to the above, as of Harch 15, 1995, calibration procedure No.
IHP 6030 IHP.014, Revision 8, for calibrating General Electric Hodel 66 type IAC induction disc current sensing relays, was not appropriate for the current level of proficiency of the II C technicians.
For example, the procedure did not define the "pick-up" current for an induction disc relay as the value at which the continuity light "just flickers."

is a Severity Level III problem (Supplement I).
